体表精确定位微创PFNA固定治疗老年股骨粗隆间骨折的临床效果及安全性研究

摘　　要：目的:研究体表精确定位微创股骨近端防旋髓内钉(PFNA)固定治疗老年股骨粗隆间骨折(ITF)的临床效果及安全性。方法:选择2018年5月-2021年4月于九江市第一人民医院治疗的92例老年ITF患者,按随机数字表法将其分为微创组与常规组,各46例。常规组采用常规开口PFNA固定治疗,微创组采用体表精确定位微创PFNA固定治疗。比较两组围手术期指标、髋关节功能及并发症。结果:微创组术中出血量少于常规组,主切口长度短于常规组(P<0.05);两组手术及住院时间、Harris评分、髋关节功能恢复优良率及并发症发生率比较,差异均无统计学意义(P>0.05)。结论:老年ITF患者采用体表精确定位微创PFNA固定治疗是安全可行的,髋关节功能恢复效果好,并可缩短切口长度、减少术中出血量,且并发症少,应用安全性较高。Objective:To investigate the clinical efficacy and safety of precise body surface positioning minimally invasive proximal femoral anti-rotation intramedullary nail (PFNA) fixation in the treatment of senile intertrochanteric fracture (ITF).Method:A total of 92 senile ITF patients treated in Jiujiang NO.1 People’s Hospital from May 2018 to April 2021 were selected and divided into the minimally invasive group and the conventional group according to random number table method,with 46 cases in each group.The conventional group was treated with conventional open PFNA fixation,while the minimally invasive group was treated with precise body surface positioning minimally invasive PFNA fixation.The perioperative indicators,hip function and complications were compared between two groups.Result:The amount of intraoperative bleeding in the minimally invasive group was less than that in the conventional group,and the length of main incision was shorter than that in the conventional group (P0.05).Conclusion:Senile ITF patients treated with precise body surface positioning minimally invasive PFNA fixation is safe and feasible,with good hip function recovery effect,shorter incision length,less intraoperative blood loss,fewer complications and higher safety.
