摘 要:目的:探讨高压氧辅助治疗中重度一氧化碳(CO)中毒患者的效果及对氧化应激指标水平的影响。方法:选取2019年9月-2021年8月莆田学院附属医院收治的121例中重度CO中毒患者,按照随机数字表法将患者分为对照组(n=60)和观察组(n=61)。对照组给予氧化樟脑注射液治疗,观察组在对照组基础上给予高压氧治疗。比较两组临床疗效、氧化应激指标[超氧化物歧化酶(SOD)、丙二醛(MDA)、神经元特异性烯醇化酶(NSE)]水平、血气指标[血氧饱和度(SaO_(2))、氧分压(PaO_(2))、二氧化碳分压(PaCO_(2))]及神经功能缺损评分(NFDS)。结果:观察组总有效率为95.08%,高于对照组的83.33%,差异有统计学意义(P<0.05)。治疗后,观察组SOD、SaO_(2)、PaO_(2)均高于对照组,MDA、NSE、PaCO_(2)、NFDS评分均低于对照组,差异均有统计学意义(P<0.05)。结论:高压氧治疗可通过调节中重度CO中毒患者氧气和二氧化碳比例,纠正氧化应激水平,从而改善神经功能,提高临床疗效。Objective:To explore the effect of adjuvant hyperbaric oxygen therapy on patients with moderate and severe carbon monoxide (CO) poisoning and its influence on the level of oxidative stress indicators.Method:A total of 121 patients with moderate and severe CO poisoning admitted to the Putian University Affiliated Hospital from September 2019 to August 2021 were selected,they were divided into control group (n=60) and observation group (n=61) according to random number table method.The control group was treated with Vitacamphorae Injection,and the observation group was treated with hyperbaric oxygen on the basis of the control group.The clinical efficacy,oxidative stress indexes[superoxide dismutase (SOD),malondialdehyde (MDA),neuron-specific enolase (NSE) levels],blood gas indexes[blood oxygen saturation (SaO_(2)),partial pressure of oxygen (PaO_(2)),partial pressure of carbon dioxide (PaCO_(2))]and neurological deficit score (NFDS) were compared between two groups.Result:The total effective rate of the observation group was 95.08%,which was higher than 83.33% of the control group,the difference was statistically significant (P<0.05).After treatment,SOD,SaO_(2) and PaO_(2) of the observation group were higher than those in the control group,MDA,NSE,PaCO_(2) and NFDS scores of the observation group were lower than those in the control group,the differences were statistically significant (P<0.05).Conclusion:Hyperbaric oxygen therapy can correct the oxidative stress level by adjusting the ratio of oxygen to carbon dioxide in patients with moderate to severe CO poisoning,thereby improving neurological function and improving clinical efficacy.
