离子温度双敏感型黏膜创面保护胶性能及黏膜修复有效性体外评价研究

摘　　要：目的为了开发一种内镜黏膜下切除术(endoscopic submucosal dissection,ESD)术后保护创面、修复黏膜损伤的生物材料,对离子温度双敏感型黏膜创面保护胶物化性能和生物学性能进行研究,评价其在ESD术后修复方面的可行性。方法采用倒瓶法、电子显微镜扫描、流变仪观察样品成胶特性、结构表征,并通过重量法对凝胶溶胀度进行评价,随后进行细胞增殖和细胞迁移实验,通过检测细胞存活率、细胞迁移率评价凝胶有效性。结果离子温度双敏感型黏膜保护胶是一种致密的无孔隙结构,在室温下呈液态,在37℃,3min迅速成胶,成胶温度在35℃左右。凝胶的最大溶胀度为238%。凝胶在浸提24 h时浸提液能够促进人正常结肠上皮细胞(human normal colon epithelial cell,CCD 841 CoN cell)增殖,促进增殖效果最佳浓度为50%;促进迁移效果具有浓度依赖性,浸提液浓度为12.5%时,培养36 h细胞迁移率达67.5%,而阴性对照迁移率仅为27.1%,具有统计学差异(P<0.001)。结论离子温度双敏感型黏膜保护胶在ESD术后修复方面有一定的作用,对其在ESD术后并发症的修复方面提供一定的参考。Objective To develop a biomaterial to protect the lesion and promote wound healing in endoscopic submucosal dissection(ESD),we study the physicochemical and biological properties of ion-temperature sensitive mucosal protective gel and to evaluate the feasibility in ESD repair.Methods We used bottle filling method,electron microscope scanning and rheometer to study the gelatinization and structure characterization of the gel.The swelling degree of the gel was evaluated by gravimetric method.Cell proliferation and cell migration experiments indicated the effectiveness of the gel.Results The ion-temperature sensitive mucosal protective gel was compact porous structure.It was liquid at room temperature and formed rapidly at 37℃in 3 min.The gel time was 35℃.The gel’s maximum swelling was 238%.When the gel was extracted for 24 h,the extract could promote the proliferation of CCD 841 CoN cells,the optimal concentration was 50%.The effect of promoting migration was concentration-dependent.When the concentration of the extract was 12.5%,the mobility of cells reached 67.5%for 36 h,while that of the negative control was only 27.1%,which had a statistical difference(P<0.001).Conclusions Ion-temperature sensitive mucosal protective gel plays a certain roal in the repair of postoperative ESD,which provides a certain reference for the repair of postoperative complications in ESD.
