摘　　要：目的探讨亚甲基四氢叶酸还原酶C677T(Methylenetetrahydrofolate reductase C677T,MTHFRC677T)基因多态性与歼击机飞行员血浆总同型半胱氨酸(totalhomocysteine,t Hcy)的关系,为飞行员高同型半胱氨酸血症(hyperhomocysteinemia,HHcy)及慢性疾病个体化防护提供临床数据。方法在小体检时,清晨空腹状态下抽取29名飞行合格飞行员的静脉血,同时检测MTHFRC677T基因及tHcy,并跟踪随访1年,对比分析不同MTHFRC677T基因型、不同年龄随访前后tHcy变化情况。结果①29名飞行员MTHFRC677T基因检测结果显示5名为CC型,14名为CT型,10名为TT型,TT型出现频率为34.48%,T等位基因出现频率高达58.62%;剔除2例(1例CT型、1例TT型)tHcy极端值后,平均tHcy为(17.96±7.63)μmol/L,CC型、CT型、TT型飞行员平均tHcy水平分别为(14.10±0.71)μmol/L、(15.59±3.43)μmol/L和(22.57±10.67)μmol/L,差异有统计学意义(F=3.546,P<0.05)。②共随访23名飞行员,随访前后平均tHcy水平分别为(17.39±7.33)μmol/L和(19.01±8.36)μmol/L,差异有统计学意义(t=-2.14,P<0.05);随访后CC型、CT型、TT型飞行员平均tHcy水平分别为(14.43±3.71)μmol/L、(17.23±3.09)μmol/L和(25.97±13.91)μmol/L,差异有统计学意义(F=3.702,P<0.05);不同基因型、不同年龄飞行员随访前后tHcy增加值差异无统计学意义;随访期内飞行时间增加值与tHcy增加值之间无明显相关性。结论MTHFRC677T基因可一定程度上预测飞行员tHcy水平,但要预测其变化规律及与tHcy的关系还需进一步研究。Objective To investigate the association between the MTHFRC677T gene polymorphism and plasmatotal homocysteine(tHcy)in fighter pilots in order to provide clinical data for treatment of pilots with hyperhomocysteinemia(HHcy)and for individualized prevention of chronic diseases.Methods A total of 29 samples of venous blood were collected frompilots qualif ied for f light during theregular physical examination.The MTHFR C677T genotypes and tHcyconcentration were analyzed,while the changes of tHcy after one year of follow-up were recorded.Besides,the changes of tHcy before and after the follow-up in different MTHFRC677T genotypes and different age groups wereanalyzed.Results①The MTHFRC677T gene test results of 29 pilots showed that 5 of these pilots were of the CC type,14 the CT type and 10 the TT type.The frequency of the TT type was 34.48%,compared with 58.62% for T allele.The average tHcy was(17.96±7.63)μmol/L after the extreme values of 2 cases(1 case of the CT type and 1 case of TT type)were excluded,but was(14.10±0.71)μmol/L,(15.59±3.43)μmol/L and(22.57±10.67)μmol/L in pilots of the CC,CT and TT type,respectively.The difference was statistically significant(F=3.546,P<0.05).②A total of 23 pilots were followed up for one year.The average tHcy of these pilots was(17.39±7.33)μmol/L before follow-up and(19.01±8.36)μmol/L after it,respectively.The difference was also statistically significant(t=-2.14,P<0.05).The average tHcy was(14.43±3.71)μmol/L,(17.23±3.09)μmol/L and(25.97±13.9)1μmol/L in pilots of the CC,CT and TT type after follow-up,respectively.The difference was statistically significant(F=3.702,P<0.05).However,there was no significant difference between different genotypes and different age groups in tHcy increment duringthe follow-up.Conclusion The MTHFRC677T gene can predict the tHcy level of pilots to some extent,but furtherresearch is needed to predict the courseof change and relationships with tHcy.
