摘　　要：目的探讨经外周穿刺中心静脉置管(PICC)化疗的白血病患者发生非计划拔管的影响因素。方法选取2018年9月至2021年4月郑州颐和医院收治的350例接受PICC置管化疗的白血病患者为研究对象,计划置管时间为12个月,术后随访,记录所有患者术后非计划拔管的发生情况。设计患者基线资料表,调查并记录研究所需资料,将可能的因素纳入,经logistic回归分析找出行PICC置管化疗白血病患者发生非计划拔管的影响因素。结果随访期间,350例患者中发生非计划拔管41例,占比11.71%(41/350)。发生组患者并发静脉炎、导管相关性血流感染、导管堵塞、并发过敏性皮炎与未发生组患者比较,差异有统计学意义(P<0.05);两组间其他基线资料比较,差异无统计学意义(P>0.05)。经logistic回归分析,结果显示,并发静脉炎、导管相关性血流感染、导管堵塞、并发过敏性皮炎是行PICC置管化疗白血病患者发生非计划拔管的危险因素(OR>1,P<0.05)。结论行PICC置管化疗白血病患者发生非计划拔管的影响因素可能为并发静脉炎、导管相关性血流感染、导管堵塞、并发过敏性皮炎。Objective To explore the influencing factors of unplanned extubation in leukemia patients with peripherally inserted central venous catheters(PICC)for chemotherapy.Methods A total of 350 leukemia patients who received PICC for chemotherapy in Zhengzhou Yihe Hospital from September 2018 to April 2021 were selected as the research subjects.The planned catheterization time was 12 months.Patients were followed up after operation,and the occurrence of unplanned extubation in all patients was recorded.The baseline data table of patients was designed,and the data required for the study were investigated and recorded.Possible factors were included,and the influencing factors of unplanned extubation in patients with leukemia undergoing PICC for chemotherapy were identified by logistic regression analysis.Results During the follow-up period,41 of the 350 patients had unplanned extubation,accounting for 11.71%(41/350).There were differences in phlebitis,catheter-related bloodstream infection,catheter blockage,and atopic dermatitis in the occurrence group and the non-occurrence group(P<0.05).There was no difference in other baseline data between the two groups(P>0.05).The results of logistic regression analysis showed that phlebitis,catheter-related bloodstream infection,catheter blockage and allergic dermatitis were the risk factors of unplanned extubation in leukemia patients with PICC for chemotherapy(OR>1,P<0.05).Conclusion The influencing factors of unplanned extubation in leukemia patients with PICC for chemotherapy may be phlebitis,catheter-related blood flow infection,catheter blockage and allergic dermatitis.
