足蹬法与杠杆复位法在肩关节脱位合并肱骨大结节骨折患者中的应用效果

摘　　要：目的探讨足蹬法、杠杆复位法应用于肩关节脱位并发肱骨大结节骨折患者的效果。方法选取遂平县中医院2019年1月至2021年12月收入治疗的70例肩关节脱位并发肱骨大结节骨折患者为研究对象,按随机数表法分为对照组(35例)和观察组(35例)。观察组接受足蹬法治疗,对照组接受杠杆复位法治疗,比较两组复位效果(复位成功率、复位时间),治疗前后健康调查简表(SF-36)评分以及术后肩关节恢复情况、并发症发生率。结果观察组的复位成功率(91.43%)高于对照组(71.43%),复位时间短于对照组,差异有统计学意义(P<0.05)。观察组功能、主动前屈活动度、前屈肌力测试、患者主观满意度及总分高于对照组,肩关节疼痛评分低于对照组,差异有统计学意义(P<0.05)。观察组术后并发症发生率(2.86%)与对照组(5.71%)比较,差异无统计学意义(P>0.05)。两组术前SF-36评分差异无统计学意义(P>0.05),两组术后SF-36评分均升高,且观察组高于对照组(P<0.05)。结论对关节脱位并发肱骨大结节骨折患者实施足蹬法治疗可提升复位成功率,缩短复位时间,改善肩关节功能与生活质量,且并发症发生率低。Objective To explore the effect of foot pedal method and lever reduction method in patients with shoulder dislocation complicated with fracture of greater tuberosity of humerus.Methods A total of 70 patients with shoulder dislocation complicated with fracture of greater tuberosity of humerus who were treated in Suiping Hospital of Traditional Chinese Medicine from January 2019 to December 2021 were selected as the research objects.They were divided into control group(35 cases)and observation group(35 cases)according to random number table method.The observation group was treated with foot pedaling method and the control group was treated with lever reduction method.The reduction effect(reduction success rate and reduction time),medical outcomes study short-form 36(SF-36)scores before and after treatment,the recovery of shoulder joint and the incidence of complications were compared between the two groups.Results The success rate of reduction in the observation group(91.43%)was higher than that in the control group(71.43%),and the reduction time was shorter than that in the control group(P<0.05).The scores of function,active flexion activity,flexion strength test,subjective satisfaction and total score in the observation group were higher than those in the control group,and the score of shoulder pain was lower than that in the control group(P<0.05).There was no difference in the incidence of postoperative complications between the observation group(2.86%)and the control group(5.71%)(P>0.05).There was no difference in preoperative SF-36 score between the two groups(P>0.05).The SF-36 score of the two groups increased after operation,and the observation group was higher than the control group(P<0.05).Conclusion The application of foot pedaling method to patients with joint dislocation complicated with fracture of greater tuberosity of humerus can improve the success rate of reduction,shorten the reduction time,improve shoulder function and quality of life,and has low incidence of postoperative complications.
