手法复位小夹板与石膏外固定治疗桡骨远端骨折对复位质量及腕关节活动度参数的影响

摘　　要：目的比较手法复位小夹板与石膏外固定治疗桡骨远端骨折对复位质量及腕关节活动度参数的影响。方法选取2019年12月至2021年12月遂平县中医院收治的118例桡骨远端骨折患者,按随机数表法分为对照组(59例,石膏外固定)与观察组(59例,手法复位小夹板)。比较两组骨折愈合情况、复位质量、腕关节Gartland-Werley(GW)功能改善情况。结果相较于对照组,观察组肿胀消失、疼痛缓解、骨折愈合与骨折线消失时间均更短,且桡骨短缩更小(P<0.05);观察组复位质量整体情况更优,优良率更高(P<0.05)。治疗前,两组患者的GW评分比较,差异无统计学意义(P>0.05),治疗后,观察组患者的GW评分低于对照组(P<0.05);治疗后,观察组患者的腕关节活动度各项参数均高于对照组(P<0.05)。结论手法复位小夹板应用于桡骨远端骨折患者可获得更为理想的效果,可促进骨折愈合,提高复位质量,改善腕关节功能及活动度,且安全性较高。Objective To compare the effects of manual reduction of small splints and plaster external fixation on the quality of reduction and the parameters of wrist range of motion in the treatment of distal radius fractures.Methods A total of 118 patients with distal radius fractures from December 2019 to December 2021 in Suiping Hospital of Traditional Chinese Medicine were selected and randomly divided into two groups according to the random number table,with 59 cases in each group.The control group was treated with plaster external fixation,and the observation group was treated with manual reduction and small splint.The fracture healing,reduction quality,wrist Gartland-Werley(GW)function score and changes in wrist range of motion parameters were compared between the two groups.Results Compared with the control group,the swelling disappearance time,pain relief time,fracture healing time and fracture line disappearance time in the observation group were shorter,and the shortening of the radius was smaller(P<0.05).The overall reduction quality of the observation group was better,the excellent and good rate was higher(P<0.05).Before treatment,there was no difference in the GW score between the two groups(P>0.05).After treatment,the GW score of the observation group was lower than that of the control group(P<0.05).After treatment,the wrist joint mobility parameters of the observation group were higher than those of the control group(P<0.05).Conclusion Manipulative reduction small splint can achieve better results in patients with distal radius fracture,which can promote fracture healing,improve reduction quality,improve wrist joint function and range of motion,and has high safety.
