多指标联合检测在特发性膜性肾病中的应用价值

摘　　要：目的分析磷脂酶A2受体(PLA2R)、免疫球蛋白G(IgG)、表皮生长因子(EGF)联合检测在特发性膜性肾病(IMN)早期诊断及预后中的评估价值。方法选取成都大学附属医院2018年6月至2020年8月收治的74例IMN患者,纳入IMN组;另选取同期健康体检者56例作为对照组。对IMN患者进行1年随访,统计预后情况。比较两组血清PLA2R、IgG及尿EGF水平,并绘制受试者工作特征曲线(ROC)分析上述因子联合检测对IMN的诊断价值。结果IMN组PLA2R、IgG水平均高于对照组,EGF水平低于对照组(P<0.05)。经1年随访,74例IMN患者中,预后不良14例(预后不良组,18.92%),预后良好60例(预后良好组,81.08%)。预后不良组PLA2R、IgG水平均高于预后良好组,EGF水平低于预后良好组(P<0.05)。ROC显示,PLA2R、IgG及EGF联合检测曲线下面积(AUC)、敏感度、特异度均高于各项指标单独检测。结论IMN患者中PLA2R、IgG及EGF存在异常表达,通过检测上述因子水平可有效评估患者预后,且联合检测可提高IMN早期诊断价值。Objective To analyze the role of combined detection of phospholipase A2 receptor(PLA2R),immunoglobulin G(IgG),and epidermal growth factor(EGF)in the early diagnosis and prognostic evaluation of idiopathic membranous nephropathy(IMN).Methods A total of 74 IMN patients admitted to the Affiliated Hospital of Chengdu University from June 2018 to August 2020 were selected and included in the IMN group.And 56 healthy volunteers undergoing physical examination during the same period were selected as the control group.The IMN patients were followed up for 1 year,and the prognosis was analyzed.Serum PLA2R,IgG and urinary EGF levels in the two groups were compared.Receiver operating characteristic curve(ROC)was drawn to analyze the diagnostic value of the combined detection of the above factors for IMN.Results Compared with the control group,the IMN group had higher PLA2R and IgG levels and lower EGF levels(P<0.05).One-year follow-up showed that among the 74 patients,14 had poor prognosis(poor prognosis group,18.92%),and 60 had good prognosis(good prognosis group,81.08%).Compared with the good prognosis group,the poor prognosis group had higher PLA2R and IgG levels and lower EGF levels(P<0.05).ROC curve showed that the area under the curve(AUC),sensitivity,and specificity of the combined detection of PLA2R,IgG,and EGF were all higher than those of each index alone.Conclusion There are abnormal expressions of PLA2R,IgG and EGF in IMN patients.Detection of the levels of the above factors can effectively assess the prognosis of IMN patients,and combined detection of the levels of the above factors can improve the value of early diagnosis of IMN.
