摘　　要：目的分析系统性红斑狼疮(systemic lupus erythematosus,SLE)患者凝血功能、狼疮抗凝物(lupus anticoagulant,LA)、血小板数量和ANA核型特征,为SLE患者临床抗凝治疗提供参考。方法选取2019年1月至2021年3月就诊于首都医科大学附属北京同仁医院的初发SLE且未发生血栓的患者79例,选取同时期50例表观健康者作为对照;分析实验组和对照组血浆凝血酶原时间(PT)、PT活动度(PT%)、国际标准化比值(INR)、活化部分凝血活酶时间(APTT)、血浆纤维蛋白原(FIB)、凝血酶时间(TT)、血浆D-二聚体(D-dimer,D-D)、纤维蛋白(原)降解产物(FDP)、狼疮抗凝物(LA)和血小板数量的差异及SLE患者ANA核型特征。结果实验组和对照组相比,PT、PT%、INR和APTT差异无统计学意义(P>0.05);TT、FIB、FDP、D-D、LA和血小板数量比较差异存在统计学意义(P<0.05);SLE患者ANA抗体阳性率为98.73%;ANA核型有胞核均质型(28.20%)、胞核斑点型(29.49%)和胞核混合型(42.31%)。结论初发SLE且未发生血栓的患者处于高凝状态,血栓形成风险高。Objective To analyze coagulation function,lupus anticoagulant,platelet count and ANA karyotype in patients with systemic lupus erythematosus in order to assist the clinical anticoagulant treatment in patients with SLE.Methods Seventy-nine outpatients and inpatients with initial SLE without thrombosis attending Beijing Tongren Hospital,Capital Medical University,from January,2019 to March,2021,were selected for the study,while 50 healthy person from the same period were selected as the control group;Coagulation indexes[PT(plasma prothrombin time),PT%(PT activity),INR(international normalized ratio),APTT(activated partial thromboplastin),FIB(plasma fibrinogen),TT(thrombin time),D-D(plasma D-dimer)and FDP(fibrin(pro)degradation product)],LA(Lupus anticoagulant),platelet count and ANA karyotype were analyzed and compared between the experimental and control groups.Results The results indicated that there were no significant differences in PT,APTT,PT%,INR,when SLE patients compared with healthy cases(P>0.05);There were differences in TT,FIB,D-D,FDP,LA and platelet count(P<0.05);The positive rate of ANA antibody in patients with SLE was 98.73%;The karyotypes of ANA were homogeneous(28.20%),spotted(29.49%)and mixed(42.31%).Conclusion Patients with initial SLE without thrombosis are in hypercoagulable state and are in high risk of thrombosis.
