摘 要:目的研究深圳地区非甲状腺疾病人群促甲状腺激素(thyroid-stimulating hormone,TSH)分泌的节律性,为甲状腺疾病的诊治提供新的思路。方法利用大数据技术挖掘2020年1月1日~2021年11月31日深圳市中医院计算机中心数据库的促甲状腺激素检验结果108071例。剔除具有明确诊断的甲状腺癌、原发性甲状腺功能亢进或甲状腺功能减退患者3393例,缺失值7289例和离群值4040例后,余下93349例为研究总体,其中男性30547例,女性62802例,平均年龄35(42±15)岁。首先,对总体进行正态检验,再根据血液样本采集时间(5:00~16:00)及时间间隔1h将其分成12组,分析TSH分泌的生物节律。多组间比较采用单因素方差分析。结果TSH值呈偏态分布。不同组间TSH中位值的最大偏差为31.4%。峰值出现于上午8:00,谷值出现于下午3:00。单因素方差分析显示,各组TSH值的差异具有统计学意义(F=99.09,P<0.01)。TSH值的周期性变化,总体上呈现上午高、下午低。结论深圳地区非甲状腺疾病人群TSH分泌具有明显的生物节律性。Objective To study the biorhythm of thyroid stimulating hormone(TSH)secretion in people with non thyroid diseases in Shenzhen,so as to provide new ideas for the diagnosis and treatment of thyroid diseases.Methods A total of 108071 cases with TSH test results were collected in the database of Computer Center of Shenzhen Traditional Chinese Medicine Hospital Affiliated to the Fourth Clinical College of Guangzhou University of Chinese Medicine from January 1,2020 to November 31,2021,excluding 3393 patients with thyroid cancer,primary hyperthyroidism or hypothyroidism,7289 missing values and 4040 outliers,the remaining 93349 patients were the study population,including 30547 males and 62802 females,with an average age of 35(42±15)years.Firstly,the normality test was excuted on the total cases,and then the blood samples were divided into 12 groups according to the sampling time(5:00~16:00)and the time interval of 1h,and the biorhythm of TSH secretion among different groups were analyzed.Comparison between multiple groups was performed by one-way ANOVA.Results The distribution of TSH values showed a skewness.The maximum deviation of median TSH between different groups was 31.4%.The acrophase appeared at 8:00 a.m.and the trough appeared at 3:00 p.m.One-way ANOVA showed that TSH values were significantly different among groups(F=99.09,P<0.01).Generally,there was a periodical variation of TSH values,higher in a.m.and lower in p.m.Conclusion There was an obvious biorhythm of TSH secretion in Shenzhen population with non-thyroid diseases.
