A型肉毒毒素联合针灸治疗脑卒中后足内翻的效果及其对足踝运动学特征的影响

摘　　要：目的 分析A型肉毒毒素联合针灸治疗脑卒中后足内翻的效果及其对足踝运动学特征的影响。方法 60例脑卒中后足内翻患者按照治疗方案的不同分为对照组(常规康复治疗)与观察组(A型肉毒毒素联合针灸),比较两组的足踝部痉挛程度(CSI评分)、步行能力(FAC分级)、下肢运动功能(FMA-LE评分)以及步态参数。结果 治疗后,观察组的CSI评分、步态周期均低于对照组,FAC分级、 FMA-LE评分、步长、步速均高于对照组(P <0.05)。结论 A型肉毒毒素联合针灸治疗脑卒中后足内翻患者的效果显著,可改善肢体痉挛情况,改善肢体运动功能以及足踝运动学特征。Objective To analyze the effect of botulinum toxin type A combined with acupuncture and moxibustion in the treatment of talipes varus after stroke and its impact on kinematic characteristics of ankle.Methods 60 patients with talipes varus after stroke were divided into the control group(routine rehabilitation therapy) and the observation group(botulinum toxin type A combined with acupuncture and moxibustion) according to different treatment plans.The degree of ankle spasm(CSI score),walking ability(FAC classification),lower extremity motor function(FMA-LE score),and gait parameters were compared between the two groups.Results After treatment,the CSI score and gait cycle of the observation group were lower than those of the control group,and the FAC classification,FMA-LE score,step length and step pace were higher than those of the control group(P <0.05).Conclusions Botulinum toxin type A combined with acupuncture and moxibustion in the treatment of patients with talipes varus after stroke has significant effect,which can improve limb spasm,and improve limb motor function and kinematic characteristics of ankle.
