摘 要:目的应用蛋白组学探讨加味四逆汤治疗缺血性中风(ischemic stroke,IS)恢复期的作用机制。方法选取60例IS恢复期阳虚证患者,随机分成两组,即对照组与治疗组每组30例,对照组给予常规基础治疗,治疗组常规基础治疗配合加味四逆汤口服,疗程为4周。治疗后予以临床疗效评定,并通过DIA蛋白组学鉴定两组间差异蛋白。结果治疗组运动功能、日常生活能力评分均有明显的改善(P<0.05),且治疗组优于对照组(P<0.05)。通过蛋白质定量筛选,两组共筛选出9个差异表达蛋白;(GO)富集分析发现差异蛋白主要富集在integrin complx、focal adhesion、integrin binding和focal adhesion assembly等。KEGG通路富集分析显示,加味四逆汤治疗IS恢复期与Tight junction、Pentose phosphate pathway、methane metabolism、axon guidance、biosynthesis of amino acids等通路密切相关。结论加味四逆汤治疗IS临床疗效显著,其作用机理可能与其调控POSTN、CD44、FA7等多个蛋白有关,并可能通过参与Tight junction、PPP等信号通路从而起到抗炎、抗氧化及神经保护等作用。Objective To explore the mechanism of modified Sini decoction(MSD)in the treatment of ischemic stroke(IS)recovery period by proteomics method.Methods 60 patients with Yang deficiency syndrome in IS recovery period were randomly divided into two groups,30 cases in each group.The control group was given conventional basic treatment,and the treatment group was given conventional basic treatment combined with MSD.The two groups were treated for acourse of 4 weeks,and the efficacy was evaluated after treatment.Differential proteins between the two groups were identified by DIA proteomics.Results①The scores of TCM syndromes,motor function and daily living ability in the treatment group were significantly improved(P<0.05),and the treatment group was better than the control group(P<0.05).②A total of nine differentially expressed proteins were screened out by protein quantitative screening.(GO)Enrichment analysis showed that the differential proteins were mainly enriched in integrin complx,focal adhesion,integrin binding,and focal adhesion assembly.The enrichment analysis of KEGG pathway showed that the recovery of IS treated with MSD was closely related to tight junction,pentose phosphate pathway,methane metabolism,axon guidance,and metabolism of amino acids.Conclusion Modified Sini Decoction has a significant clinical effect in the treatment of IS,and its mechanism may be related to POSTN,CD44,FA7 and other proteins.It may participate in tight junction,PPP and other signaling pathways to play anti-inflammatory,anti-oxidant and neuroprotective effects.
