摘 要:目的评价2017—2020年四川省部分地区6~59月龄儿童肠道病毒71型(EV-A71)灭活疫苗肠道病毒71型疫苗的保护效果(VE)。方法从《中国疾病预防控制信息系统》和《四川省免疫规划信息管理系统》中获取2017—2020年四川省部分市、县6~59月龄儿童个案,采用检测阴性设计的1∶2配对病例对照研究方法进行调查分析。结果病例组纳入308例,对照组匹配616例,全程两剂次EV-A71灭活疫苗接种VE(93%,95%CI:87%~96%)高于单剂次接种EV-A71灭活疫苗的VE(78%,95%CI:51%~90%);24~59月龄组单剂次接种EV-A71灭活疫苗的VE(92%,95%CI:26%~99%)高于6~23月龄组(72%,95%CI:33%~88%);接种EV-A71灭活疫苗后到发病,不同时间间隔,VE值不同,最后1次接种EV-A71灭活疫苗到病例发病的时间间隔为12~17月的疫苗保护率最高。结论接种EV-A71灭活疫苗可有效预防EV-A71感染的手足口病,全程两剂次接种的保护效果优于单剂次接种。Objective To evaluate the vaccine efficacy(VE)of the inactivated Enterovirus type 71(EV-A71)vaccine for children aged 6-59 months in some areas of Sichuan Province from 2017 to 2020.Methods Cases of children aged 6 to 59 months were obtained in some cities and counties of Sichuan Province from 2017 to 2020 from the“China Disease Prevention and Control Information System”and“Sichuan Province Immunization Program Information Management System”,a 1∶2 matched case-control study method with a negative test design was used for investigation and analysis.Results Overall vaccine efficacy was estimated to be 93%[95%CI:87%-96%]for fully vaccinated children and 78%[95%CI:51%~90%]for partially vaccinated children.The vaccine efficacy for partial vaccination was 92%[95%CI:26%-99%]in children aged 24-59 months and 72%[95%CI:33%-88%]in children aged 6-23 months.Conclusions The Inactivated Enterovirus type 71(EV-A71)vaccine was effective in preventing hand,foot,and mouth disease due to EV-A71 virus infection for children aged 6-59 months.
