骨间背侧动脉逆行岛状皮瓣修复糖尿病患者手背创面的疗效分析

摘　　要：目的 分析骨间背侧动脉逆行岛状皮瓣修复糖尿病患者手背创面的疗效。方法 自2017年1月至2020年3月,上海市第一人民医院创伤中心采用骨间背侧动脉逆行岛状皮瓣修复18例糖尿病患者手背创面,记录患者的性别、年龄、侧别、致伤原因、创面面积、随访时间、手术时间和住院时间等资料。观察皮瓣成活情况和皮瓣感觉恢复情况。采用数字评分法评估患者满意度。对皮瓣成活情况及感觉恢复情况进行统计学分析。结果 18例患者获随访10~18个月,皮瓣完全成活17例(94.4%),皮瓣远端部分坏死1例(5.6%)。皮瓣感觉恢复情况:S1级10例、S2级7例、S2+级1例。平均手术时间(56.7±11.2)min,平均住院时间(16.9±2.7)d,患者满意度评分为(8.9±0.7)分。结论 骨间背侧动脉逆行岛状皮瓣是修复糖尿病患者手背创面的较好选择。Objective To analyze the efficacy of reverse posterior interosseous island flap in the repair of wounds of dorsal hand in patients with diabetes. Methods From January 2017 to March 2020, a total of 18 patients with diabetes were selected from our department. The reverse posterior interosseous island flap was used to repair the wounds of dorsal hand. The gender, age, affected side, pathogenesis, wound size, follow-up duration, operative time and hospitalization time of patients were recorded. The survival and sensory recovery of flap were observed. The patient satisfaction was assessed by numerical scoring. Results A total of 18 patients were followed up for 10 to18 months. The flap survived completely in 17 cases(94.4%), and the distal part of flap was necrotic in 1 case(5.6%). The sensory recovery result was grade S1 in 10 cases, grade S2 in 7 cases and grade S2+ in 1 case. The average operative time was(56.7±11.2) minutes and the average hospitalization time was(16.9±2.7) days. The patient satisfaction score was(8.9±0.7). Conclusion The reverse posterior interosseous island flap is a good choice for the repair of wounds of dorsal hand in patients with diabetes.
