阿昔洛韦联合地塞米松治疗早期眼部带状疱疹的效果分析

摘　　要：目的探讨早期眼部带状疱疹患者采用阿昔洛韦联合地塞米松治疗的效果。方法80例早期眼部带状疱疹患者,采用随机数字表法分为对照组和观察组,每组40例。对照组患者采用阿昔洛韦治疗,观察组患者采用阿昔洛韦联合地塞米松治疗。对比两组临床疗效,症状缓解时间(止痛时间、止疱时间、疱疹结痂时间),治疗前后疼痛、睡眠状况评分及炎症因子水平[白细胞介素-1β(IL-1β)、β-内啡肽、肿瘤坏死因子-α(TNF-α)]、细胞免疫功能指标(CD3^(+)、CD4^(+)、CD8^(+))水平、生活质量评分,不良反应发生情况。结果观察组患者总有效率为97.50%,高于对照组的82.50%,差异具有统计学意义(P<0.05)。观察组患者止痛时间、止疱时间、疱疹结痂时间分别为(1.64±0.42)、(2.04±0.37)、(3.54±0.38)d,均短于对照组的(4.98±1.01)、(5.84±1.28)、(5.89±1.04)d,差异具有统计学意义(P<0.05)。治疗后,两组患者疼痛、睡眠状况评分均低于治疗前,观察组患者疼痛评分(1.49±0.37)分、睡眠状况评分(2.14±0.23)分均低于对照组的(2.39±0.32)、(4.29±0.38)分,差异具有统计学意义(P<0.05)。治疗后,两组患者的IL-1β、TNF-α水平低于治疗前,β-内啡肽水平高于治疗前,观察组患者的IL-1β(2.41±0.38)μg/L、TNF-α(6.54±1.68)μg/L均低于对照组的(4.35±0.72)、(8.59±1.95)μg/L,β-内啡肽(50.71±5.49)ng/ml高于对照组的(43.22±5.53)ng/ml,差异具有统计学意义(P<0.05)。治疗后,两组患者的CD3^(+)、CD4^(+)水平均高于治疗前,CD8^(+)水平低于治疗前,且观察组患者的CD3^(+)(60.79±2.74)%、CD4^(+)(36.53±4.07)%均高于对照组的(53.82±2.49)%、(33.17±3.65)%,CD8^(+)(26.19±1.84)%低于对照组的(28.37±2.15)%,差异具有统计学意义(P<0.05)。治疗后,两组患者的生活环境、生理、社会关系、心理评分均高于治疗前,且观察组患者的生活环境评分(28.71±3.58)分、生理评分(24.45±1.12)分、社会关系评分(10.87±0.6Objective To discuss the therapeutic effect of acyclovir combined with dexamethasone in the treatment of early ocular herpes zoster.Methods A total of 80 patients with early ocular herpes zoster were divided into control group and observation group by random numerical table,with 40 cases in each group.Patients in the control group were treated with acyclovir,and patients in the observation group were treated with acyclovir combined with dexamethasone.Both groups were compared in terms of clinical efficacy,symptom relief time(pain-relieving time,blister-stopping time and herpes scabbing time),pain and sleep status scores,and levels of inflammatory factors[interleukin-1β(IL-1β),β-endorphin,tumor necrosis factor-α(TNF-α)],levels of cellular immune function indexes(CD3^(+),CD4^(+),CD8^(+)),quality of life scores before and after treatment,and occurrence of adverse reactions.Results The total effective rate in the observation group was 97.50%,which was higher than 82.50% in the control group,and the difference was statistically significant(P<0.05).The pain-relieving time,blister-stopping time and herpes scabbing time in the observation group were(1.64±0.42),(2.04±0.37)and(3.54±0.38)d,respectively,which were shorter than(4.98±1.01),(5.84±1.28)and(5.89±1.04)d in the control group,and the differences were statistically significant(P<0.05).After treatment,the pain and sleep status scores of the two groups were lower than those of this group before treatment,and the pain score(1.49±0.37)points and the sleep status score(2.14±0.23)points of the observation group were lower than(2.39±0.32)and(4.29±0.38)points of the control group.All the differences were statistically significant(P<0.05).After treatment,the levels of IL-1βand TNF-αin the two groups were lower than those of this group before treatment,and the level ofβ-endorphin was higher than that of this group before treatment;the IL-1β(2.41±0.38)μg/L and TNF-α(6.54±1.68)μg/L in the observation group were lower than(4.35±0.72)and(8.59±1.95)μg/L i
