摘  要:目的分析二维超声“三切面”法联合时空关联成像(STIC)在胎儿复杂心脏畸形检查中的可行性。方法450例孕妇为研究对象,均接受二维超声“三切面”法检查、STIC检查。所有胎儿均在之后随访中被证实:心内膜垫缺损2例、单腔心2例、心脏位置异常1例、左心室双出口1例、左心室发育不良2例、右心室双出口1例、永存左上腔静脉8例,复杂心脏畸形共17例。对比二维超声“三切面”法、二维超声“三切面”法联合STIC的胎儿复杂心脏畸形诊断准确率。结果二维超声“三切面”法检出心内膜垫缺损1例、单腔心0例、心脏位置异常1例、左心室双出口1例、左心发育不良0例、右心室双出口0例、永存左上腔静脉4例,胎儿复杂心脏畸形诊断准确率为41.18%;二维超声“三切面”法联合STIC检出心内膜垫缺损2例、单腔心2例、心脏位置异常1例、左心室双出口0例、左心发育不良1例、右心室双出口1例、永存左上腔静脉7例,胎儿复杂心脏畸形诊断准确率为82.35%。二维超声“三切面”法联合STIC的胎儿复杂心脏畸形诊断准确率高于二维超声“三切面”法,差异具有统计学意义(P<0.05)。结论二维超声“三切面”法联合STIC进行胎儿复杂心脏畸形检查可有效提升疾病诊断准确率,有临床应用价值。Objective To analyze the feasibility of the two dimensional ultrasound"threesection"method combined with spatio-temporal image correlation(STIC)in examining complex fetal cardiac malformations.Methods A total of 450 pregnant women were selected as the research subjects,and they all received the two-dimensional ultrasound"three-section"method and STIC examination.All fetuses were confirmed to have endocardial cushion defect in 2 cases,single-chamber heart in 2 cases,abnormal heart position in 1 case,left ventricle double outlet in 1 case,left ventricle double outlet in 2 cases,right ventricle double outlet in 1 case,permanent left superior vena cava in 8 cases,and complex cardiac malformation in 17 cases at followup.The diagnostic accuracy of complex fetal cardiac malformation by two-dimensional ultrasound"three-section"method,two-dimensional ultrasound"three-section"method combined with STIC was compared.Results Twodimensional ultrasound"three-section"method detected endocardial cushion defect in 1 case,single-chamber heart in 0 case,abnormal heart position in 1 case,left ventricle double outlet in 1 case,left ventricle double outlet in 0 case,right ventricle double outlet in 0 case,permanent left superior vena cava in 4 cases,with diagnostic accuracy of complex fetal cardiac anomalies as 41.18%.Two-dimensional ultrasound"three-section"method combined with STIC detected endocardial cushion defect in 2 cases,single-chamber heart in 2 cases,abnormal heart position in 1 case,left ventricle double outlet in 0 case,left ventricle double outlet in 1 case,right ventricle double outlet in 1 case,permanent left superior vena cava in 7 cases,with diagnostic accuracy of complex fetal cardiac anomalies as 82.35%.The diagnostic accuracy of complex fetal cardiac malformations diagnosed by twodimensional ultrasound"three-section"method combined with STIC was higher than that of two-dimensional ultrasound"three-section"method,and the difference was statistically significant(P<0.05).Conclusion Twodimensional ultrasound"three-sect
