自拟痛经汤联合艾灸治疗寒凝血瘀型原发性痛经的疗效观察

摘　　要：目的观察自拟痛经汤联合艾灸对寒凝血瘀型原发性痛经患者痛经评分及血清前列腺素F_(2α)(PGF_(2α))的影响。方法97例寒凝血瘀型原发性痛经患者为研究对象,随机分为治疗组(48例)和对照组(49例)。其中治疗组脱落6例,完成42例;对照组脱落10例,完成39例。治疗组采用自拟痛经汤联合艾灸治疗,对照组采用屈螺酮炔雌醇片(商品名:优思明)治疗。对比两组治疗效果及治疗前、治疗3个月后、随访6个月后痛经程度、痛经证候评分、PGF_(2α)水平。结果治疗组治疗总有效率95.2%高于对照组的79.5%,差异有统计学意义(P<0.05)。随访6个月后,治疗组视觉模拟评分法(VAS)评分(3.17±1.48)分低于对照组的(3.97±1.53)分,差异有统计学意义(P<0.05)。治疗3个月后,治疗组痛经证候评分(1.25±1.26)分低于对照组的(1.99±1.90)分,差异有统计学意义(P<0.05);随访6个月后,治疗组痛经证候评分(2.06±1.40)分低于对照组的(2.95±1.82)分,差异有统计学意义(P<0.05)。随访6个月后,治疗组PGF_(2α)水平(54.24±4.19)pg/m低于对照组的(57.69±6.63)pg/ml,差异有统计学意义(P<0.05)。结论自拟痛经汤联合艾灸可有效改善寒凝血瘀型原发性痛经患者的临床症状,降低血清PGF_(2α)水平,值得推广。Objective To observe the effect of self-made Tongjing Decoction combined with moxamoxibustion on dysmenorrhea score and serum prostaglandin F_(2α)(PGF_(2α))of primary dysmenorrhea with cold blood coagulation and blood stasis syndrome.Methods A total of 97 cases of primary dysmenorrhea with cold blood coagulation stasis syndrome were selected as the research subjects,and were randomly divided into treatment group(48 cases)and control group(49 cases).Among them,6 cases in the treatment group dropped out and 42 cases were completed;10 cases in the control group dropped out and 39 cases were completed.The treatment group was treated with self-made Tongjing Decoction and moxa-moxibustion,and the control group was treated with drospirenone and ethinylestradiol tablets(trade name:Yasmin).The therapeutic effect and the level of dysmenorrhea,dysmenorrhea syndrome score and PGF_(2α) level before treatment,after 3 months of treatment,and after 6 months of follow-up were compared between the two groups.Results The total effective rate 95.2% of the treatment group was higher than 79.5% of the control group,and the difference was statistically significant(P<0.05).After 6 months of follow-up,the visual analogue scale(VAS)score(3.17±1.48)points of the treatment group was lower than(3.97±1.53)points of the control group,and the difference was statistically significant(P<0.05).After 3 months of treatment,the dysmenorrhea syndrome score(1.25±1.26)points of the treatment group was lower than(1.99±1.90)points of the control group,and the difference was statistically significant(P<0.05).After 6 months of follow-up,the dysmenorrhea syndrome score(2.06±1.40)points in the treatment group was lower than(2.95±1.82)points in the control group,and the difference was statistically significant(P<0.05).After 6 months of follow-up,the level of PGF_(2α)(54.24±4.19)pg/ml in the treatment group was lower than(57.69±6.63)pg/ml in the control group,and the difference was statistically significant(P<0.05).Conclusion Self-made Tongjing Deco
