加味脱敏煎联合莲藤消炎止痒散治疗小儿湿疹(湿热浸淫型)的临床观察

摘　　要：目的观察加味脱敏煎联合莲藤消炎止痒散治疗小儿湿疹(湿热浸淫型)的临床疗效。方法96例湿疹(湿热浸淫型)患儿,随机分为治疗组(50例)和对照组(46例)。对照组患儿口服西替利嗪联合外涂炉甘石制剂进行治疗,治疗组患儿口服加味脱敏煎联合外洗莲藤消炎止痒散进行治疗。比较两组患儿治疗后临床疗效,治疗前后血清学指标[白细胞介素-2(IL-2)、白细胞介素-4(IL-4)、肿瘤坏死因子-α(TNF-α)]水平,治疗前后T淋巴细胞亚群(CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+))水平。结果治疗后,治疗组患儿的临床总有效率为96.00%,明显高于对照组的78.26%,差异具有统计学意义(P<0.01)。治疗后,治疗组患儿的IL-2(220.41±17.03)pg/ml明显高于对照组的(152.26±18.96)pg/ml,IL-4(31.40±4.32)pg/ml、TNF-α(6.60±1.35)pg/ml明显低于对照组的(39.42±5.02)、(10.40±5.33)pg/ml,差异均具有统计学意义(P<0.01)。治疗后,治疗组患儿的CD4^(+)(38.80±5.36)%、CD4^(+)/CD8^(+)(1.59±0.22)明显高于对照组的(34.56±5.26)%、(1.20±0.20),CD8^(+)(24.40±4.53)%明显低于对照组的(28.32±4.32)%,差异均具有统计学意义(P<0.01)。结论加味脱敏煎联合莲藤消炎止痒散可有效改善湿疹(湿热浸淫型)患儿临床症状及细胞因子水平,调节免疫功能,疗效可靠安全。Objective To observe the clinical efficacy of modified Tuomin decoction combined with Lianteng Xiaoyan Zhiyang powder in the treatment of infantile eczema(dampness-heat immersion syndrome).Methods A total of 96 cases of infantile eczema(dampness-heat immersion syndrome)were randomly divided into treatment group(50 cases)and control group(46 cases).Children in the control group were treated with oral administration of cetirizine and topical application of calamine preparations,and children in the treatment group were treated with oral modified Tuomin decoction and topical application of Lianteng Xiaoyan Zhiyang powder.Both groups were compared in terms of clinical efficacy after treatment,and serological indexes[interleukin-2(IL-2),interleukin-4(IL-4),tumor necrosis factor-α(TNF-α)]before and after treatment,T lymphocyte subsets(CD4^(+),CD8^(+),CD4^(+)/CD8^(+))levels before and after treatment.Results After treatment,the total clinical effective rate in the treatment group was 96.00%,which was significantly higher than 78.26% in the control group,and the difference was statistically significant(P<0.01).After treatment,the IL-2(220.41±17.03)pg/ml in the treatment group was significantly higher than(152.26±18.96)pg/ml in the control group,and the IL-4(31.40±4.32)pg/ml,TNF-α(6.60±1.35)pg/ml were significantly lower than(39.42±5.02)and(10.40±5.33)pg/ml in the control group.All the differences were statistically significant(P<0.01).After treatment,the CD4^(+)(38.80±5.36)%,CD4^(+)/CD8^(+)(1.59±0.22)in the treatment group were significantly higher than(34.56±5.26)%,(1.20±0.20)in the control group,and the CD8^(+)(24.40±4.53)% was significantly lower than(28.32±4.32)% in the control group.All the differences were statistically significant(P<0.01).Conclusion The efficacy of modified Tuomin Decoction combined with Lianteng Xiaoyan Zhiyang powder in the treatment of infantile eczema(dampness-heat immersion syndrome)is reliable and safe,and can effectively improve clinical symptoms and cytokine levels,and regula
