摘  要:目的:探讨血小板减少性紫癜患者血小板计数(PLT)、D-二聚体、骨髓巨核细胞计数水平的变化及临床意义。方法:选取42例血小板减少性紫癜患者作为研究组,另收集42例健康体检者作为对照组。检测两组人群的PLT、D-二聚体及骨髓巨核细胞计数水平,采用Logistic回归筛选血小板减少性紫癜的危险因素,并绘制PLT、D-二聚体、骨髓巨核细胞计数联合检测血小板减少性紫癜的受试者工作特征曲线(ROC)。结果:研究组的PLT低于对照组,而D-二聚体、骨髓巨核细胞计数水平则高于对照组,差异有统计学意义(P<0.05);一般资料分析结果显示研究组和对照组年龄、性别、体重指数及居住地等资料比较差异无统计学意义(P>0.05),而PLT、D-二聚体、骨髓巨核细胞计数等资料比较差异有统计学意义(P<0.05);多因素Logistic回归分析的结果显示,PLT、D-二聚体、骨髓巨核细胞计数等是血小板减少性紫癜的影响因素(P<0.05)。PLT、D-二聚体、骨髓巨核细胞计数联合检测血小板减少性紫癜的ROC曲线分析的结果显示,PLT、D-二聚体、骨髓巨核细胞计数的AUC相比于单个指标有明显提升,差异有统计学意义(P<0.05);联合检测的灵敏度和特异性优于单个指标检测,差异有统计学意义(P<0.05)。结论:PLT、D-二聚体、骨髓巨核细胞计数等是血小板减少性紫癜的影响因素,且PLT、D-二聚体、骨髓巨核细胞计数联合检测血小板减少性紫癜的检测效能优于单独检测。Objective To investigate the changes and clinical significance of PLT,D-dimer and bone marrow megakaryocyte counts in patients with thrombocytopenic purpura.Method 42 patients with thrombocytopenic purpura were selected as the research group,and 42 healthy subjects were collected as the control group.Detect the platelet count(PLT),D-dimer and bone marrow megakaryocyte count levels of the two groups of people,use Logistic regression to screen the risk factors of thrombocytopenic purpura,and plot the combination of PLT,D-dimer,and bone marrow megakaryocyte counts Detect the ROC curve of thrombocytopenic purpura.Results The PLT of the study group was lower than that of the control group,while the counts of D-dimer,bone marrow megakaryocytes were higher than that of the control group(P<0.05),while statistics such as PLT,D-dimer,and bone marrow megakaryocyte counts existed.Academic difference(P<0.05).The results of multivariate logistic regression analysis showed that PLT,D-dimer,and bone marrow megakaryocyte count were the influencing factors of thrombocytopenic purpura(P<0.05).The results of ROC curve analysis of PLT,D-dimer,and bone marrow megakaryocyte count combined to detect thrombocytopenic purpura showed that the AUC of PLT,D-dimer,and bone marrow megakaryocyte count were significantly improved compared to a single index(P<0.05).The sensitivity and specificity of the combined detection are 90.50% and 95.20%,which are better than a single index detection(P<0.05).Conclusion PLT,D-dimer,bone marrow megakaryocyte count,etc.are the influencing factors of thrombocytopenic purpura,and the combined detection efficiency of PLT,D-dimer,and bone marrow megakaryocyte count is better than the detection efficiency of thrombocytopenic purpura alone Detection.
