摘　　要：目的:分析经尿道前列腺等离子电切术(TURP)治疗老年高危大体积前列腺增生(BPH)的疗效及对机体炎性反应的影响。方法:选择172例老年高危大体积BPH患者,均予以TURP术治疗,统计手术指标以及并发症发生情况,比较治疗前后尿道功能恢复情况、国际前列腺症状评分(IPSS)、炎性反应指标。结果:172例患者平均手术时间(78.26±5.62)min、膀胱冲洗时间(1.25±0.16)d、尿管留置时间(2.56±0.33)d、住院时间(11.26±1.95)d、术中出血量(87.26±10.62)ml。其中1例出现尿路刺激征,1例出现尿失禁,并发症发生率为1.16%。术后3 d残余尿量低于术前,术后3 d最大尿流速高于术前,差异均有统计学意义(P<0.05)。术后3 d IPSS评分低于术前,差异均有统计学意义(P<0.05)。术后3 d血清降钙素原(PCT)、C-反应蛋白(CRP)、淀粉样蛋白A(SAA)、白细胞介素-6(IL-6)均低于术前,差异均有统计学意义(P<0.05)。结论:TURP术应用于老年高危大体积BPH治疗中,创伤性较小,术后患者机体恢复速度快,炎性反应较轻,并发症较少。Objective To analyze the efficacy of TURP(transurethral resection of the prostate) in the treatment of high-risk large-volume prostatic hyperplasia(BPH) in the elderly and its influence on the body’s inflammatory response.Method 172 elderly patients with high-risk and large-volume BPH were selected,they were treated with TURP,surgical indicators and complications were calculated,and the recovery of urethral function,IPSS score and Inflammatory response indicators before and after treatment were compared.Results The average operation time of 172 patients was(78.26±5.62) min,the bladder irrigation time was(1.25±0.16) d,the urinary catheter indwelling time was(2.56±0.33) d,the hospitalization time was(11.26±1.95) d,intraoperative bleeding The amount was(87.26±10.62) ml.Among them,1 case developed urinary tract irritation and 1 case developed urinary incontinence.The complication rate was 1.16%.The residual urine volume at 3 days after operation was lower than that before operation,and the maximum urine flow rate at 3 days after operation was higher than that before operation(P<0.05).The 3 d IPS score after operation was lower than that before operation( P<0.05).Serum PCT,CRP,SAA and IL-6 were all lower than those before operation 3 days after operation(P<0.05).Conclusion TURP is used in the treatment of high-risk and large-volume BPH in the elderly.It is less traumatic,and the patient’s body recovers quickly after the operation,with less inflammation and fewer complications.
