摘  要:目的:探索肾炎防衰液改善慢性肾功能衰竭大鼠肾性贫血的作用机制。方法:32只雄性SPF级Wistar大鼠随机分为对照组、模型组、肾炎防衰液组、罗沙司他组,每组8只,除对照组外,其余3组采用左侧肾切除联合腺嘌呤灌胃方法制作肾性贫血模型,根据组别分别以去离子水、肾炎防衰液、罗沙司他灌胃,12周后,分别检测血常规、肾功能、铁代谢指标及血清炎症因子,Western blot检测肾组织脯氨酰羟化酶2(PHD-2)、低氧诱导因子2α(HIF-2α)蛋白表达。结果:与模型组比较,肾炎防衰液能够显著降低慢性肾功能衰竭大鼠尿素氮、血肌酐水平(P<0.01),升高血中红细胞、血红蛋白、血清铁、铁蛋白水平(P<0.01),降低大鼠血清铁调素、白细胞介素-6、肿瘤坏死因子α水平(P<0.01),上调肾组织HIF-2α蛋白表达(P<0.01)。结论:肾炎防衰液能够在改善慢性肾功能衰竭大鼠肾功能同时,能够通过改善铁代谢紊乱状态,降低血清铁调素以及炎症因子水平,上调HIF-2α蛋白表达改善肾性贫血。Objective: To explore the mechanism of Shenyan Fangshuai Decoction for improving renal anemia in rats with chronic renal failure. Methods: Thirty-two male SPF Wistar rats were randomly divided into control group, model group,Shenyan Fangshuai Decoction group and Roxadustat group, 8 rats in each group. In addition to the control group, the remaining three groups were constructed to renal anemia model with the left renal resection combined adenine gavage. According to the groups, the rats were fed with deionized water, Shenyan Fangshuai Decoction, Roxadustat, respectively. After 12 weeks, blood routine, renal function, iron metabolism index and inflammation factors were tested, respectively. The expression of PHD-2,HIF-2α of renal tissues were tested by Western blot. Results: Compared with model group, Shenyan Fangshuai Decoction could significantly reduce SCR and BUN levels in rats with chronic renal failure(P<0.01), increase the level of serum RBC, HGB,SI and FER(P<0.01), decrease the level of serum hepcidin, IL-6 and TNF-α(P<0.01), and up-regulate protein expression of HIF-2αin the kidney tissues(P<0.01). Conclusion: Shenyan Fangshuai Decoction can improve renal function in rats with chronic renal failure, and improve renal anemia by improving the disorder of iron metabolism, reducing the levels of serum hepcidin and inflammatory factors, and up-regulating the protein expression of HIF-2α.
