摘　　要：膝骨关节炎(KOA)是一种常见于中老年人的慢性进行性骨关节疾病,以疼痛、肿胀、功能障碍等为主要临床表现。在中医学中,膝骨关节炎属于“膝痹病”“骨痹”“痹证”的范畴,《黄帝内经》认为本病的病因病机与“骨、筋、肌肉”均相关,基于“骨、筋、肌肉”理论进行针灸结合运动疗法治疗KOA能够有效地缓解疼痛、肿胀,改善关节功能。文章从“骨、筋、肌肉”理论与膝痹的病因病机出发,阐释针灸及运动疗法干预膝痹的内在机制,对今后针灸结合运动疗法治疗KOA的临床研究提出改进与完善,以期为KOA的治疗提供新的思路与方法。Knee osteoarthritis(KOA) is a chronic progressive bone and joint disease commonly seen in middle-aged and elderly people, with pain, swelling and dysfunction as the main clinical manifestations. In traditional Chinese medicine, KOA belongs to the category of ‘knee Bi’, ‘bone Bi’ and ‘Bi syndrome’. Huangdi Neijing believes that the etiology and pathogenesis of this disease are related to ‘bone, tendon and muscle’. Based on ‘bone, tendon and muscle theory’, acupuncture combined with exercise therapy to treat KOA can effectively relieve pain, swelling and improve joint function. Based on the theory of bone,tendon and muscle and the etiology and pathogenesis of knee arthralgia, this paper explains the internal mechanism of acupuncture and moxibustion and exercise therapy in the intervention of KOA, and puts forward the improvement and perfection of clinical studies on the treatment of knee osteoarthritis by acupuncture and moxibustion combined with exercise therapy in the future, in order to provide new ideas and methods for the treatment of KOA.
