摘  要:目的:通过观察脾虚血脂异常患者高密度脂蛋白(HDL)功能及炎症因子的变化,研究脾虚膏脂转输障碍对血脂异常疾病的影响,探讨中医药防治血脂异常疾病的作用机制。方法:在2018年8-10月期间,于沈阳医学院附属第二医院门诊招募并筛选脾虚血脂异常患者50例为治疗组,予以香砂六君丸观察4周。同时纳入30名健康志愿者作为健康对照组,全自动生化分析仪检测各组人群血清总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)水平,ELISA法检测各组人群血清中血清淀粉样蛋白A(SAA)、载脂蛋白AⅠ(ApoA-Ⅰ)、卵磷脂胆固醇酰基转移酶(LCAT)、磷脂转运蛋白(PLTP)、胆固醇酯转移蛋白(CETP)、NLR家族PYRIN域蛋白3(NLRP3)、白细胞介素-1β(IL-1β)、肿瘤坏死因子-α(TNF-α)水平。结果:与健康对照组比较,治疗组TC、TG、LDL-C、SAA、PLTP、CETP、NLRP3、IL-1β、TNF-α水平显著增高(P<0.05),HDL-C、ApoA-Ⅰ、LCAT显著降低(P<0.05);与治疗前比较,香砂六君丸治疗后TC、TG、LDL-C、SAA、PLTP、CETP、NLRP3、IL-1β、TNF-α水平显著降低(P<0.05),HDL-C、ApoA-Ⅰ、LCAT显著升高(P<0.05)。结论:脾虚血脂异常患者HDL功能失调导致血清炎症因子水平升高。香砂六君丸可纠正HDL失功能状态,降低脾虚血脂异常患者血清炎症反应,调控胆固醇逆转运相关因子进而改善血脂水平。Objective: By observing the changes of high density lipoprotein(HDL) function and inflammatory factor levels in patients with spleen deficiency dyslipidemia, to investigate the effects of impaired lipids transfer of spleen deficiency on dyslipidemia diseases and explore the mechanism of traditional Chinese medicine in preventing and treating dyslipidemia diseases. Methods: From August to October 2018, 50 patients with spleen deficiency and dyslipidemia were recruited and selected as the experimental group in the outpatient department of the Second Affiliated Hospital of Shenyang Medical College, and were observed with Xiangsha Liujun Pills for 4 weeks. At the same time, 30 healthy volunteers were included as the control group.Automatic biochemical analyzer was used to detect serum TC, TG, HDL-C and LDL-C levels in all groups. The serum levels of SAA, ApoA-Ⅰ, LCAT, PLTP, CETP, NLRP3, IL-1β and TNF-α were measured by ELISA. Results: Compared with the control group, the levels of TC, TG, LDL-C, SAA, PLTP, CETP, NLRP3, IL-1β and TNF-α in the dyslipidemia group with spleen deficiency were significantly increased(P<0.05), while the levels of HDL-C, ApoA-Ⅰ and LCAT were significantly decreased(P<0.05). Compared with before treatment, the serum levels of TC, TG, LDL-C, SAA, PLTP, CETP, NLRP3, IL-1βand TNF-α were significantly decreased after the intervention of Xiangsha Liujun Pills(P<0.05), while the levels of HDL-C,ApoA-Ⅰ and LCAT were significantly increased(P<0.05). Conclusion: Dysfunctional HDL in patients with spleen deficiency and dyslipidemia leads to increased levels of serum inflammatory factors. Xiangsha Liujun Pills can correct HDL dysfunction, reduce the inflammatory reaction of the patients with spleen deficiency and dyslipidemia, regulate cholesterol reverse transport related factors and improve blood lipid levels.
