摘 要:目的探讨粪钙卫蛋白对过敏性紫癜患儿消化道损伤的预警价值。方法选择2019年10月至2022年1月在桂林市人民医院儿科住院诊治的首次发病的70例过敏性紫癜患儿作为研究对象,按是否有消化道受累分为腹型过敏性紫癜组(42例)和非腹型过敏性紫癜组(28例)。其中腹型过敏性紫癜组按病情轻重分为腹型轻症组(24例)和腹型重症组(18例),选择同期在本院象山社区服务中心体检的50例健康儿童作为健康对照组。采用酶联免疫吸附试验(ELESA)测定所有患儿和健康组儿童的粪钙卫蛋白水平进行分析。结果过敏性紫癜患儿的粪钙卫蛋白水平高于健康对照组,腹型过敏性紫癜组患儿的粪钙卫蛋白水平高于非腹型过敏性紫癜组,腹型重症组患儿的粪钙卫蛋白水平高于腹型轻症组,差异均有统计学意义(P<0.05)。结论过敏性紫癜患儿的粪钙卫蛋白水平与健康儿童存在差异,检测粪钙卫蛋白水平有助于早期识别过敏性紫癜患儿是否存在消化道损伤并评判其严重程度,有一定的预警价值,可为临床及早干预提供依据。Objective To investigate the early warning value of fecal calprotectin for gastrointestinal injury in children with Henoch-Schonlein purpura.Methods A total of 70 cases of children with Henoch-Schonlein purpura who were hospitalized in Department of Pediatrics,Guilin People’s Hospital for the first time from October 2019 to January 2022 were selected as study subjects,and they were divided into abdominal Henoch-Schonlein purpura group(42 cases)and non-abdominal Henoch-Schonlein purpura group(28 cases)according to whether they had digestive tract involvement or not.The abdominal Henoch-Schonlein purpura group was divided into the abdominal mild group(24 cases)and the abdominal severe group(18 cases)according to the severity of the disease,and 50 healthy children who were examined in Xiangshan Community Service Center of our hospital at the same time were selected as the healthy control group.The fecal calprotectin levels of all children with Henoch-Schonlein purpura and healthy children were analyzed by enzyme linked immunosorbent assay(ELESA).Results Children with Henoch-Schonlein purpura had higher fecal calprotectin levels than the healthy control group,children in the abdominal Henoch-Schonlein purpura group had higher fecal calprotectin levels than the non-abdominal Henoch-Schonlein purpura group,the fecal calprotectin levels of children in the severe abdominal Henoch-Schonlein purpura group were higher than those in the mild group,and the differences were statistically significant(P<0.05).Conclusion The fecal calprotectin levels in children with Henoch-Schonlein purpura are different from those in healthy children.The detection of fecal calprotectin levels can help identify the presence of gastrointestinal injury in children with Henoch-Schonlein purpura at an early stage and assess its severity,which has a certain early warning value and can provide a basis for early clinical intervention.
