创伤性脑损伤病人感知批评、家庭情感表达水平与心理社会功能的相关性

摘　　要：目的:探讨创伤性脑损伤病人感知批评、家庭情感表达水平与心理社会功能的关系。方法:选取2019年1月—2020年1月在我院首诊治疗的创伤性脑损伤病人135例。采用感知批评测量问卷(PCM)、家庭问卷(FQ)及Rivermead脑损伤随访问卷(RHFUQ)对其进行调查。结果:创伤性脑损伤病人FQ得分为(14.11±1.50)分,23例(17.0%)病人家庭成员属于高度家庭情感表达批评。PCM感知批评得分为(4.31±1.35)分,33例(24.4%)病人属于高度感知批评;批评敏感性得分为(4.40±1.48)分,36例(26.7%)病人感知到高度批评敏感性。RHFUQ得分为(16.86±11.78)分。创伤性脑损伤病人感知批评、批评敏感性得分及FQ得分与RHFUQ得分均呈正相关(均P<0.001)。多元线性回归分析结果显示:交通事故、格拉斯哥评分、高度家庭情感表达批评、高度感知批评及高度批评敏感性是创伤性脑损伤病人RHFUQ得分的独立影响因素。结论:创伤性脑损伤病人感知批评、家庭情感表达水平与其心理社会功能存在关联,医务人员应实施相关干预措施,以改善病人的心理社会功能。Objective:To explore the relationship between perceived criticism,family emotional expression level and psychosocial function in patients with traumatic brain injury.Methods:A total of 135 patients with traumatic brain injury who were first diagnosed and treated in our hospital from January 2019 to January 2020,were selected as subjects.Investigation was carried out by the Perceived Criticism Measure(PCM),the Family Questionnaire(FQ),and the Rivermead Head Injury Follow⁃Up Questionnaire(RHFUQ).Results:The FQ score of patients with traumatic brain injury was 14.11±1.50.There were 23 family members of patients with high degree of family emotion and criticism,accounting to 17.0%.The PCM's perceived criticism score was 4.31±1.35.There were 33 patients with high degree of criticism,accounting to 24.4%.The criticism sensitivity score was 4.40±1.48.There were 36 patients with high degree of criticism sensitivity,accounting to 26.7%.The RHFUQ score was 16.86±11.78.There were significant positively correlation between FQ scores,PCM's perceived criticism score and criticism sensitivity score(all P<0.001).The results of multiple linear regression analysis showed that traffic accidents,GCS score,high family emotional expression criticism,high perceived criticism and high criticism sensitivity were the independent influencing factors of RHFUQ score in patients with traumatic brain injury.Conclusions:The level of perceived criticism and family emotional expression in patients with traumatic brain injury were correlated with their psychosocial function.Medical staff should implement relevant interventions to improve the psychosocial functioning of patients.
