摘　　要：目的 探讨子宫内膜异位症(EM)患者血清C-C基序趋化因子配体28(CCL28)的表达及其对在位子宫内膜间质细胞上皮间质转化(EMT)的影响。方法 用酶联免疫吸附实验(ELISA)检测30例EM患者(EM组)和子宫肌瘤患者(对照组)血清CCL28表达水平。用受试者操作特征(ROC)曲线分析血清CCL28诊断EM的价值,计算曲线下面积(AUC)。从EM患者子宫内膜分离并培养原代间质细胞,稳定感染sh-RNA-CCL28慢病毒的细胞记为sh-CCL28组;感染sh-RNA阴性对照慢病毒的细胞记为sh-NC组。Western-blot检测EMT相关分子:E-钙黏蛋白(E-cadherin)、N-钙黏蛋白(N-cadherin)、波形纤维蛋白(Vimentin)、果蝇蜗牛同源蛋白(Snail);Transwell法检测细胞侵袭和迁移;Annexin-V-FITC/PI流式细胞术检测细胞凋亡。结果 EM组血清CCL28浓度高于对照组(P<0.05)。血清CCL28诊断EM的AUC为0.813,灵敏度和特异度分别为83.33%和73.33%。与sh-NC组相比,sh-CCL28组E-cadherin蛋白表达水平较高,而N-cadherin、Vimentin、Snail蛋白表达水平较低(P<0.05)。与sh-NC组相比,sh-CCL28组细胞侵袭数和迁移数较低,而凋亡率较高(P<0.05)。结论 EM患者血清CCL28表达水平升高,对临床诊断有一定价值;CCL28通过调控EMT影响EM的发生发展。Objective To investigate the expression of serum C-C motif chemokine ligand 28(CCL28) in patients with endometriosis(EM) and its effect on epithelial-mesenchymal transition(EMT) of eutopic endometrial stromal cells.Methods The expression levels of serum CCL28 in 30 EM patients(EM group) and uterine fibroids(control group) were detected by enzyme-linked immunosorbent assay(ELISA). The value of serum CCL28 in the diagnosis of EM was analyzed by receiver operating characteristic(ROC) curve, and AUC was calculated. Primary stromal cells were isolated and cultured from the endometrium of EM patients, and cells stably infected with sh-RNA-CCL28 lentivirus were designated as sh-CCL28 group.Cells infected with sh-RNA negative control lentivirus were designated as sh-NC group. Western blot detection of EMTrelated molecules: E-cadherin, N-cadherin, Vimentin, Snail, Transwell assay cell invasion and migration, apoptosis was detected by Annexin-V-FITC/PI flow cytometry. Results The serum CCL28 concentration in the EM group was higher than that in the control group(P<0.05). The AUC of serum CCL28 in the diagnosis of EM was 0.813, and the sensitivity and specificity were 83.33% and 73.33%, respectively. Compared with sh-NC group, the expression level of E-cadherin protein in sh-CCL28 group was higher, while the protein expression levels of N-cadherin, Vimentin and Snail were lower(P<0.05).Compared with the sh-NC group, the sh-CCL28 group had lower numbers of cell invasion and migration, and higher apoptotic rate(P<0.05). Conclusion The expression level of serum CCL28 in patients with EM is increased, which has a certain value for clinical diagnosis. CCL28 affects the occurrence and development of EM by regulating EMT.
