四子散热敷结合康复训练治疗脑卒中上肢痉挛性偏瘫的临床效果

摘　　要：目的探讨四子散热敷结合康复训练治疗脑卒中上肢痉挛性偏瘫的临床效果。方法选取2019年9月至2021年9月在遵义医科大学第五附属(珠海)医院康复医学科住院的60例脑卒中上肢痉挛性偏瘫患者作为研究对象,采用随机数字表法将其分为试验组和对照组,每组各30例。对照组患者采用常规康复训练,试验组患者在常规康复训练的基础上联合应用四子散热敷治疗,疗程均为3周。比较两组患者的上肢痉挛程度、Fugl-Meyer运动功能评定量表上肢部分(FMA-U)评分及临床疗效。结果两组患者干预前的上肢痉挛程度及FMA-U评分比较,差异无统计学意义(P>0.05);试验组干预3周后的上肢痉挛程度轻于对照组,差异有统计学意义(P<0.05);试验组干预3周后的FMA-U评分高于对照组,差异有统计学意义(P<0.05);试验组患者的临床治疗总有效率高于对照组,差异有统计学意义(P<0.05)。结论四子散热敷结合康复训练可以改善脑卒中后上肢痉挛性偏瘫患者的痉挛程度,同时改善上肢运动功能,疗效均优于常规康复训练。Objective To investigate the clinical effect of Sizi Powder hot compress combined with rehabilitation training in the treatment of upper limb spastic hemiplegia after stroke.Methods A total of 60 stroke patients with upper limb spastic hemiplegia who were admitted to the Department of Rehabilitation Medicine of The Fifth Affiliated(Zhuhai)Hospital of Zunyi Medical University from September 2019 to September 2021 were selected as the subjects.They were divided into experimental group and control group by random number table method,with 30 patients in each group.Patients in the control group received routine rehabilitation training,and patients in the experimental group were combined with Sizi Powder hot compress treatment on the basis of routine rehabilitation training.The course of treatment was 3 weeks.The degree of upper limb spasm,the changes of the upper limb part of the Fugl-Meyer motor function scale for upper 1imb(FMA-U)score and the clinical efficacy were compared between the two groups.Results There were no significant differences in the degree of upper limb spasm and FMA-U score between the two groups before intervention(P>0.05).After 3 weeks of intervention,the degree of upper limb spasm in experimental group was lighter than that in control group,and the difference was statistically significant(P<0.05).The FMA-U score of experimental group after 3 weeks of intervention was higher than that of control group,and the difference was statistically significant(P<0.05).The total effective rate of clinical treatment in experimental group was higher than that in control group,the difference was statistically significant(P<0.05).Conclusion Sizi Powder hot compress combined with rehabilitation training can improve the spasm degree of spasmodic hemiplegia of upper limbs after stroke,and improve the upper limb motor function,the curative effect is better than conventional rehabilitation training.
