血清C反应蛋白、中性粒细胞与淋巴细胞比值与尿毒症腹膜透析患者发生感染性腹膜炎的相关性

摘　　要：目的探讨血清C反应蛋白(CRP)、中性粒细胞与淋巴细胞比值(NLR)与尿毒症腹膜透析(PD)患者发生感染性腹膜炎(SP)的相关性。方法用方便抽样方式选择2018年1月至2020年12月于龙岩市第二医院行PD治疗的196例尿毒症患者作为研究对象,按是否发生SP将其分为两组,42例发生SP的患者为观察组,154例未发生SP的患者为对照组。比较两组患者的血清CRP、NLR水平,分析血清CRP、NLR水平与SP发生的相关性,以及CRP、NLR评估SP的价值。结果观察组患者的血清CRP、NLR水平高于对照组,差异有统计学意义(P<0.05)。血清CRP、NLR水平与SP发生呈正相关(P<0.05)。CRP诊断SP的曲线下面积(AUC)为0.838(95%置信区间为0.760~0.916),敏感度为0.806,特异度为0.833;NLR诊断SP的AUC为0.867(95%置信区间为0.786~0.948),敏感度为0.871,特异度为0.858。结论血清CRP、NLR水平与尿毒症PD患者SP的发生有关。Objective To investigate the correlation between serum C-reactive protein(CRP),neutrophil-to-lymphocyte ratio(NLR)and the occurrence of infectious peritonitis(SP)in uremic peritoneal dialysis(PD)patients.Methods A total of 196 patients with uremia received PD treatment in the Second Hospital of Longyan City from January 2018 to December 2020 were selected as the research objects,and they were divided into two groups according to whether they had SP or not,and 42 patients with SP were set as the observation group,154 patients without SP were set as the control group.The convenient sampling method was used.The serum CRP and NLR levels of the two groups were compared,and the correlation between the serum CRP and NLR levels and the occurrence of SP was analyzed,as well as the value of CRP and NLR in evaluating SP.Results The levels of serum CRP and NLR in the observation group were higher than those in the control group,and the difference was statistically significant(P<0.05).Serum CRP and NLR levels were positively correlated with the occurrence of SP(P<0.05).The area under curve(AUC)of CRP for the diagnosis of SP was 0.838(95%confidence interval:0.760-0.916),the sensitivity was 0.806,and the specificity was 0.833;the AUC of NLR for diagnosing SP was 0.867(95%confidence interval:0.786-0.948),the sensitivity was 0.871,and the specificity was 0.858.Conclusion Serum CRP and NLR levels are closely related to the occurrence of SP in uremic PD patients.
