古代湖湘医派医家辨治癫狂病处方用药规律分析

摘　　要：目的通过数据挖掘探寻湖湘医派古代医家对癫狂病的理、法、方、药等的临床经验和用药特色。方法使用软件SPSS 18.0、SPSS Modeler 18.0进行聚类分析和关联规则分析,统计《湖湘名医典籍精华丛书》中治疗癫狂病的高频药物、高频药物功效分类、性味归经、高频聚类、关联药组、复杂网络关联,总结其用药规律和相关治法。结果最终纳入《湖湘名医典籍精华丛书》中治疗癫狂病的处方共67首,共147味药。频次≥5的药物共有33味,共计使用293次,占所有药物使用频次的59.19%。功效主要以补虚药、清热药和化痰止咳平喘药等11类药为主,药性主要涉及寒、温、平性药,药味主要涉及苦、甘、辛,脏腑归经频次统计显示累及主要为肺、脾、心、胃、肝、肾。药物关联规则分析得到15个核心药对,其中“甘草-当归”药对支持度最高。主要以甘草、半夏、茯苓、陈皮等药进行配伍,与高频聚类分析结果中的C7聚类药物一致。高频聚类分析共得到9个新药物组合。结论湖湘医派古代医家治疗癫狂病以化痰泻火为主,除邪务尽,又兼以扶正为本。另需分经而治,五脏分治,也可五脏合治,重视愈后。Objective Through data mining,the clinical experience and medication characteristics of the ancient doctors of Huxiang medical school on the theory,method,prescription and medicine of insanity were explored.Methods The software SPSS 18.0,SPSS Modeler 18.0 were used for cluster analysis and association analysis,to make a statistical analysis on high-frequency drugs,common drug efficacy,sexual and flavor meridians,high-frequency clustering,related drug groups and complex network correlation,and summarize their medication laws and related treatment methods in the Essence of Huxiang Famous Physicians′Books.Results A total of 67 prescriptions for the treatment of madness were included in the Essence of Huxiang Famous Physicians′Books,with a total of 147 herbs.There were 33 drugs with a frequency of≥5,which were used a total of 293 times,accounting for 59.19%of the frequency of use of all drugs.Their efficacy is mainly based on 11 types of medicines such as tonic,heat-clearing,phlegm-relieving,cough-relieving and asthma-relieving medicines.Their properties mainly involve cold,warm and mild drugs,and their tastes mainly involve bitter,sweet and pungent.Statistics on the frequency of meridians return of organs show that they mainly involve lung,spleen,heart,stomach,liver and kidney.Fifteen core drug pairs were obtained by drug association rule analysis,among which"Licorice-Angelica"drug pair had the highest support.Licorice,Poria cocos,Tangerine peel,Pinellia ternata and other drugs were mainly used for compatibility,which was consistent with the C7 clustering drugs in the high-frequency cluster analysis results.Nine new drug combinations were obtained by high-frequency cluster analysis.Conclusion Ancient doctors of Huxiang medical school had attached importance to treat madness mainly by resolving phlegm and purging fire,eliminating evil,and also focusing on strengthening the righteousness.In addition,they needed to treat the meridians separately,treat the five zang organs separately,or treat the five zang organs
