云探视提高急诊重症监护室患者满意度研究调查分析

摘　　要：目的探讨云探视在急诊重症监护室(EICU)患者探视管理中的应用效果。方法选取2020年1月至12月九江市第一人民医院采用常规床边预约式探视模式期间EICU收治的265例患者作为对照组,另选取2021年1月至12月九江市第一人民医院采用云探视模式期间EICU收治的患者265例作为观察组。比较两组医院感染发生率、ICU获得性衰弱、谵妄发生率、家属心理状态、患者满意度和护患纠纷发生率。结果两组探视前家属心理状态比较,差异无统计学意义(P>0.05)。观察组呼吸机相关肺炎、留置导尿管相关性感染、导管相关性血性感染发生率均低于对照组,差异有统计学意义(P<0.05)。观察组谵妄、ICU获得性衰弱的总发生率低于对照组,差异有统计学意义(P<0.05)。观察组探视后焦虑自评量表(SAS)评分、抑郁自评量表(SDS)评分均低于对照组,差异有统计学意义(P<0.05)。观察组家属满意度高于对照组,护患纠纷发生率低于对照组,差异有统计学意义(P<0.05)。结论在EICU患者探视管理中采取云探视,能够减少感染和谵妄发生,保证患者就医安全,有效减轻患者家属不良情绪,减少护患矛盾,从而提高患者满意度。Objective To explore the application effect of cloud visitation in emergency intensive care unit(EICU)patient visitation management.Methods A total of 265 patients admitted to the EICU during the period of the routine bedside appointment visit mode in Jiujiang First People′s Hospital from January to December 2020 were selected as the control group,and 265 patients admitted to the EICU during the cloud visit mode in Jiujiang First People′s Hospital from January to December 2021 were selected as the observation group.The incidence of nosocomial infection,ICU-acquired asthenia,the incidence of delirium,the psychological status of family members,patient satisfaction and the incidence of nurse-patient disputes were compared between the two groups.Results There was no significant difference in the psychological state of family members between the two groups before visiting(P>0.05).The incidence rates of ventilator-related pneumonia,indwelling catheter-related infection and catheter-related blood infection in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).The total incidence of ICU-acquired weakness and delirium in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).The scores of self-rating depression scale(SDS)and self-rating depression scale(SDS)in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).The family satisfaction of the observation group was higher than that of the control group,and the incidence of nurse-patient disputes was lower than that of the control group,with statistical significances(P<0.05).Conclusion Adopting cloud visitation in the visit management of EICU patients can reduce the incidence of infection and delirium,ensure the safety of patients seeking medical treatment,effectively reduce the negative emotions of patients′families,reduce the contradiction between nurses and pa
