不育男性精子DNA碎片指数与年龄、精子顶体酶活性以及精液常规参数的相关性分析

摘　　要：目的:探讨不育男性精子DNA碎片指數(DFI)与男性年龄、精子顶体酶活性以及精液常规參数之间的相关性。方法:收集2020年1月~2021年6月在生殖医学门诊就诊的不育男性患者资料,筛选出精液进行了精子DNA碎片指數,精子顶体酶活性以及精液常规全套检查的患者为研究对象。按照男性的年龄分成3组:<30岁组(1组),30~39岁组(2组),≥40岁组(3组)。将3组之间的精子DNA碎片指数(DFI),精子浓度,精子活力(前向运动精子百分率),正常形态精子比例以及精子顶体酶活性进行相关性分析。结果:不有男性患者年龄与精子DFI之间、男性患者年龄与精子密度之间呈现正相关;年龄与精子活力、正常形态精子比例之间和精子DFI与精子活力、正常形态精子比例之间及精子顶体酶活性与精子密度之间呈现负相关;年龄与精子顶体酶活性之间和精子顶体酶活性与精子DFI、精子活力、正常形态精子比例之间无明显相关性。结论:年龄对男性生育能力有显著的影响,随着男性年龄增加,精子DFI显著增加,精子活力、正常形态精子比例显著降低。因此,建议有生育需求的大龄(≥40岁)男性尽早到相关机构进行生育咨询与评估。Objective:To investigate the correlation between sperm DNA fragmentation index(DFI)and male age,sperm acrosin enzyme activity and semen routine parameters in infertile meiL Methods:The data of infertile male patients who visited the reproductive medicine clinic from January 2020 to June 2021 were collected,and the patients whose semen underwent sperm DNA fragmentation index,sperm acrosin enzyme activity and routine full set of semen examinations were selected as the research objects.The males were divided into 3 groups according to their age:<30 years old group(group 1),30~39 years old group(group 2),and ≥40 years old group(group 3).The sperm DNA fragmentation index(DFI),sperm concentration,sperm motility(percentage of forward motile spern),proportion of sperm with normal morphology and sperm acrosin activity were correlated among the three groups.Results:There was a positive correlation between the age of infertile men and sperm DFI,and between the age of men and sperm density;the relationship between age and sperm motility,the proportion of normal sperm,and the relationship between sperm DFI and sperm motility,and the proportion of normal sperrrL There was a negative correlation between sperm acrosin activity and sperm density there was no significant correlation between age and sperm acrosin activity or between sperm acrosin activity and sperm DFI,sperm motility,and proportion of normal sperm.Con elusion:Age has a significant effect on male fertility.With the increase of male age,sperm DFI increases significantly,while sperm motility and proportion of sperm with normal morphology decrease significantly.Therefore,it is recommended that older men(≥40 years old)with,fertility needs go to relevant institutions for fertility consultation and evaluation as soon as possible.
