摘 要:目的:探讨内质网应激蛋白激酶R样内质网激酶(PERK)和CCAAT/增强子结合蛋白同源蛋白(CHOP)在甲亢性肌病中的作用。方法:选取2020年2月至2021年6月在南宁市第一人民医院初次诊断的急性甲亢性肌病患者21例,单纯甲亢患者30例,同时选取健康者25例作为正常对照组。比较3组血清促甲状腺素(TSH)、游离甲状腺素(FT_(4))、游离三碘甲状腺原氨酸(FT_(3))和促甲状腺受体抗体(TRAb)水平,提取外周静脉血RNA,采用实时荧光定量PCR(RT-qPCR)检测PERK、CHOP mRNA的表达水平。结果:与单纯甲亢组比较,TRAb、FT_(4)、FT_(3)、PERK、CHOP mRNA水平在急性甲亢性肌病组中增高(P<0.01)。在急性甲亢性肌病中,TRAb与PERK、CHOP mRNA表达水平呈正相关关系(r=0.898,P<0.01;r=0.897,P<0.01)。结论:内质网应激PERK、CHOP可能在急性甲亢性肌病发病的过程中起着重要的作用,TRAb结合PERK、CHOP有利于早期判断急性甲亢性肌病。Objective:To investigate the role of protein kinase R-like endoplasmic reticulum kinase(PERK)and CCAAT/homologous protein(CHOP)of endoplasmic reticulum stress(ERS)in hyperthyroid myopathy.Methods:Twenty-one patients with acute hyperthyroid myopathy and 30 patients with simple hyperthyroidism in The First People’s Hospital of Nanning from February 2020 to June 2021 were selected as subjects,and 25 healthy persons were selected as normal control group.The levels of serum thyroid stimulating hormone(TSH),free thyroxine(FT_(4)),free triiodothyronine(FT_(3))and thyrotropin receptor artibody(TRAb)were compared among the three groups.RNA was extracted from peripheral venous blood.The expression levels of PERK and CHOP mRNA were detected by real-time fluorescence quantitative PCR(RT-qPCR).Results:Compared with simple hyperthyroidism group,the levels of TRAb,FT_(4),FT_(3),PERK and CHOP mRNA in acute hyperthyroid myopathy group were higher(P<0.01).In acute hyperthyroid myopathy,there was a positive correlation between TRAb and the expression levels of PERK and CHOP mRNA(r=0.898,P<0.01;r=0.897,P<0.01).Conclusion:ERS PERK and CHOP may play an important role in the pathogenesis of acute hyperthyroid myopathy.TRAb combined with PERK and CHOP is helpful for early diagnosis of acute hyperthyroid myopathy.
