探讨原发性膝关节退行性变(OA)程度与半月板损伤程度的相关性研究

摘　　要：目的研究半月板损伤程度与原发性膝关节退行性变(OA)程度的相关性。方法选取2018年9月—2021年6月江苏省南京市高淳人民医院收治的296例原发性OA患者为研究对象,所有患者均行常规磁共振(MRI)检查,评估患者半月板损伤程度、膝关节退行性变程度,并对半月板损伤程度与膝关节退行性变程度的相关性进行分析。结果296例患者均行MRI扫描,左膝关节139例(46.96%),右膝关节157例(153.04%);膝关节退行性变程度为Ⅰ级2例(0.68%),Ⅱ级21例(7.09%),Ⅲ级44例(14.86%),Ⅳ级229例(77.37%);半月板损伤程度为0级0例,Ⅰ级63例(21.28%),Ⅱ级81例(27.37%),Ⅲ级152例(51.35%)。以半月板损伤部位为依据,将296例患者分为4组,内侧半月板前角损伤26例(8.78%),内侧半月板后角损伤161例(51.39%),外侧半月板后角损伤87例(29.39%),外侧半月后前角损伤22例(7.43%)。OA退变Ⅰ级与半月板损伤之间无相关性;OA退变Ⅱ级、Ⅲ级及Ⅳ级与半月板损伤程度之间具有相关性,OA程度越明显,半月板损伤概率越大,且损伤程度越明显(r=3.231);损伤概率比较,内侧半月板后角损伤>内侧半月板前角损伤>外侧半月板后角损伤>外侧半月板前角损伤。结论OA患者常伴有半月板损伤,以内侧半月板损伤最为常见,且半月板损伤程度与OA程度呈正相关关系。Objective To study the correlation between the degree of meniscus injury and the degree of primary knee degeneration(OA).Methods A total of 296 patients with primary OA admitted to Nanjing Gaochun People's Hospital of Jiangsu Province from September 2018 to June 2021 were selected as the research objects.All patients underwent routine magnetic resonance imaging(MRI).The degree of meniscus injury and the degree of knee joint degeneration were evaluated.The correlation between the degree of meniscus injury and the degree of knee joint degeneration was analyzed.Results MRI scans were performed in all 296 patients,139 cases(46.96%)of the left knee and 157 cases(53.04%)of the right knee.The degree of knee joint degeneration was gradeⅠin 2 cases(0.68%),gradeⅡin 21 cases(7.09%),gradeⅢin 44 cases(14.86%),and gradeⅣin 229 cases(77.37%).The degree of meniscus injury was grade 0 in 0 cases,gradeⅠin 63 cases(21.28%),gradeⅡin 81 cases(27.37%),and gradeⅢin 152 cases(51.35%).According to the injury site of the meniscus,296 patients were dⅣided into 4 groups,26 patients(8.78%)were injured in the anterior horn of the medial meniscus,and 161 patients(51.39%)were injured in the posterior horn of the medial meniscus.The posterior horn of the lateral meniscus was injured in 87 cases(29.39%),and the posterior an⁃terior horn of the lateral meniscus was injured in 22 cases(7.43%).There was no correlation between gradeⅠOA degeneration and meniscus damage;there was a correlation between gradeⅡ,ⅢandⅣOA degeneration and the degree of meniscus damage.The more obvious the de⁃gree of OA,the greater the probability of meniscus damage,and the degree of injury was more obvious(r=3.231);the comparison of injury probability showed that the injury of the posterior horn of the medial meniscus>the injury of the anterior horn of the medial meniscus>the injury of the posterior horn of the lateral meniscus>the injury of the anterior horn of the lateral meniscus.Conclusion OA patients are often accompanied by meniscus injury,an
