关节镜与切开复位Herbert钉内固定术治疗桡骨小头骨折患者的效果比较

摘　　要：目的:比较关节镜与切开复位Herbert钉内固定术治疗桡骨小头骨折患者的效果。方法:选取2018年8月至2020年7月该院收治的82例桡骨小头骨折患者进行前瞻性研究,按随机数字表法将其分为对照组与观察组各41例。对照组行切开复位Herbert钉内固定术治疗,观察组行关节镜Herbert钉内固定术治疗,比较两组治疗效果、不同时间疼痛程度[视觉模拟评分法(VAS)]评分、治疗前后肘关节功能[Mayo肘关节功能(MEPS)]评分和并发症发生率。结果:观察组治疗总有效率为95.12%(39/41),高于对照组的75.61%(31/41),差异有统计学意义(P<0.05);术后14 d,观察组VAS评分低于对照组,差异有统计学意义(P<0.05);术后3个月,观察组MEPS评分高于对照组,差异有统计学意义(P<0.05);观察组并发症发生率为4.88%,低于对照组的29.27%,差异有统计学意义(P<0.05)。结论:关节镜Herbert钉内固定术治疗桡骨小头骨折患者可提高治疗总有效率和MEPS评分,降低VAS评分和并发症发生率,其效果优于切开复位Herbert钉内固定术治疗。Objective:To compare effects of arthroscopic and open reduction and Herbert screw internal fixation in treatment of patients with radial head fractures.Methods:82 patients with radial head fractures who were treated in our hospital from August 2018 to July 2020 were selected for the prospective study,and were divided into control group and observation group according to random number table method,41 cases in each group.The control group was treated with open reduction and Herbert screw internal fixation,while the observation group was treated with arthroscopic Herbert screw internal fixation.The treatment effects,the pain scores at different times[visual analogue scale(VAS)],the elbow function[Mayo elbow performance scale(MEPS)]score,and the incidence of complications were compared between the two groups before and after the treatment.Results:The total effective rate of treatment in the observation group was 95.12%(39/41),which was higher than the control group of 75.61%(31/41),and the difference was statistically significant(P<0.05).On the 14th day after the surgery,the VAS score of the observation group was lower than that of the control group,and the difference was statistically significant(P<0.05).3 months after the surgery the MEPS score of the observation group was higher than that of the control group,and the difference was statistically significant(P<0.05).Further,the incidence of complications in the observation group was 4.88%,which was lower than 29.27%in the control group,and the difference was statistically significant(P<0.05).Conclusions:Arthroscopic Herbert screw internal fixation in the treatment of radial head fractures can improve the total effective rate of treatment and the MEPS score and reduce the VAS score and the incidence of complications.Moreover,it is superior to open reduction and Herbert nail internal fixation.
