炎症角化型汗孔角化症1例  

Inflammatory Keratosis Porokeratosis:A Case Report

摘　　要：患者女,62岁。左小腿下段红斑伴轻度瘙痒18年。皮肤科情况:左小腿下段一纵行分布的椭圆形红色斑片,境界清楚,表面角化粗糙,呈鲜红色,周围呈褐色,无明显隆起,无糜烂、渗出。皮损组织病理示:表皮角化过度,局部表皮凹陷处有角化不全柱,下方无颗粒层,可见角化不良细胞。真皮浅层少量淋巴细胞浸润,可见噬色素细胞。实验室检查无异常。诊断:炎症角化型汗孔角化症。治疗:给予阿维A胶囊口服,外用复方氟米松软膏、咪喹莫特乳膏,3周后有所好转。现在随访中。A 62-year-old female patient presented with erythema on the lower left leg accompanied by mild pruritus for 18 years.Physical examination of the department of dermatology showed that there was a longitudinal oval bright red patch in the lower left calf with brown surrounding.The boundary of the rash was clear,and the surface was rough and keratinized,without obvious eminence,erosion and exudation.Histopathology showed hyperkeratosis and keratinized column and the granule layer disappeared among which some parakeratotic cells could be found.A few lymphocytes infiltrated in the superficial layer of dermis,and pigment phagocytes were observed.The inflammatory keratosis of porokeratosis was diagnosed.The patient was treated with acitretin capsule,compound flamethasone ointment and imiquimod cream.The lesions improved after three weeks of treatment and had been following up.
