平衡针灸联合滋阴泻火方治疗阴虚火旺型顽固性失眠临床研究  

Clinical Study on Balance Acupuncture Combined with Ziyin Xiehuo Prescription for Intractable Insomnia of Yin Deficiency with Effulgent Fire Type

摘　　要：目的:观察平衡针灸联合滋阴泻火方治疗阴虚火旺型顽固性失眠的临床疗效。方法:选取84例阴虚火旺型顽固性失眠患者,采用随机数字表法分为观察组和对照组各42例。对照组给予艾司唑仑片治疗,观察组在对照组的基础上加用平衡针灸联合滋阴泻火方治疗。2组均持续治疗20 d,比较2组治疗前后中医证候积分、睡眠质量[匹兹堡睡眠质量指数(PSQI)、失眠严重程度指数(ISI)]、焦虑状况[广泛性焦虑障碍量表(GAD-7)],并评估治疗后持续性注意力测验(CPT)[反应时间分测验(RT)、X分测验(XT)、AX分测验(AXT)]的漏答率、误答率及平均反应时间。结果:治疗后,2组虚烦难寐、心悸怔忡、五心烦热、平素焦虑等证候评分及总分均较治疗前明显降低(P<0.05),且观察组各项证候评分及总分均低于对照组(P<0.05)。治疗后,2组PSQI、ISI评分均较治疗前明显降低(P<0.05),且观察组PSQI、ISI评分均低于对照组(P<0.05)。治疗后,2组GAD-7评分较治疗前明显降低(P<0.05),且观察组GAD-7评分显著低于对照组(P<0.05)。治疗后,观察组在RT、XT、AXT测试中的漏答率及平均反应时间均明显小于对照组(P<0.05);观察组RT、XT测试中的误答率均明显低于对照组(P<0.05);2组间AXT测试的误答率比较,差异无统计学意义(P>0.05)。结论:平衡针灸联合滋阴泻火方治疗可有效改善阴虚火旺型顽固性失眠患者的睡眠质量及持续性注意力。Objective:To observe the clinical effect of balance acupuncture combined with Ziyin Xiehuo prescription for intractable insomnia of yin deficiency with effulgent fire type.Methods:A total of 84 cases of patients with intractable insomnia of yin deficiency with effulgent fire type were divided into the observation group and the control group according to the random number table method,with 42 cases in each group.The control group was treated with estazolam tablets,and the observation group was additionally treated with balance acupuncture combined with Ziyin Xiehuo prescription based on the treatment of the control group.Both groups were treated for 20 days.Before and after treatment,the Chinese medicine symptom scores,sleep quality including Pittsburgh Sleep Quality Index(PSQI)and Insomnia Severity Index(ISI),and anxiety including Generalized Anxiety Disorder Scale-7(GAD-7)in the two groups were compared.After treatment,the omission rates,commission rates and mean response time of continuous performance test(CPT)[reaction test(RT)、X test(XT)、AX test(TXT)]were recorded.Results:After treatment,the symptom scores of vexation of deficiency type leading to insomnia,severe palpitations,vexing heat in the five centers and usual anxiety as well as the total scores in the two groups were significantly decreased when compared with those before treatment(P<0.05),and the above symptom scores and the total score in the observation group were lower than those in the control group(P<0.05).After treatment,the scores of PSQI and ISI in the two groups were significantly decreased when compared with those before treatment(P<0.05),and the scores of PSQI and ISI in the observation group were lower than those in the control group(P<0.05).After treatment,the GAD-7 scores in the two groups were significantly decreased when compared with those before treatment(P<0.05),and the score in the observation group was significantly lower than that in the control group(P<0.05).After treatment,the omission rate and mean response time in the te
