盐水鼻腔冲洗联合多种药物治疗变应性鼻炎的网状Meta分析

摘　　要：目的运用网状Meta分析的方法评价盐水鼻腔冲洗联合多种药物治疗变应性鼻炎(AR)的效果。方法计算机检索PubMed、Embase、Cochrane Library、中国知网、万方数据知识服务平台、维普数据库和中国生物医学文献数据库,搜集单独使用不同浓度、不同容量的盐水冲洗鼻腔或联合药物使用治疗AR的随机对照试验,检索时限为建库至2021年4月7日。由2名研究者独立筛选文献、提取资料并评价纳入研究的偏倚风险后,采用Stata 15.0软件进行网状Meta分析。结果最终共纳入23个随机对照试验,包括2287例患者。网状Meta分析结果显示,在改善患者鼻部症状评分方面,联合用药时高渗极大容量盐水+糖皮质激素药物效果可能优于其他治疗措施;单纯使用盐水冲洗时,等渗大容量盐水鼻腔冲洗效果可能最优。结论当前证据表明,联合用药时,高渗极大容量盐水+鼻部糖皮质激素在改善成人鼻部症状评分效果优于其他;单独使用盐水冲洗时,建议使用等渗大容量盐水进行鼻腔冲洗。Objective To evaluate the effect of saline nasal irrigation combined with multiple drugs in the treatment of allergic rhinitis(AR)using a network Meta-analysis.Methods Randomized controlled trials of nasal irrigation with different concentrations and volumes of saline alone or in combination with drugs for AR were searched by computer in PubMed,Embase,Cochrane Library,China National Knowledge Infrastructure(CNKI),WanFang Data,VIP Database and China Biomedical Literature Database.The retrieval time limit was from the establishment of the database to April 7,2021.Two investigators independently screened articles,extracted data,and assessed the risk of bias of included studies.A network Meta-analysis was performed using Stata 15.0 software.Results A total of 23 randomized controlled trials including 2287 patients were finally included.The results of the network Meta-analysis showed that in improving the nasal symptom score of the patients,the combination of hypertonic ultra-high volume saline and glucocorticoid drugs was more effective than other treatments when using drugs in combination.Isotonic high-volume saline nasal irrigation worked best when using saline irrigation alone.Conclusions Existing evidence shows that when using drugs in combination,hypertonic ultra-high volume saline combined with intranasal corticosteroids(INS)drugs are more effective than others in improving nasal symptom scores in adults,and when saline irrigation is used alone,it is recommended to use isotonic high-volume saline for nasal irrigation.
