摘 要:目的:探讨抗阻运动加速导管相关性血栓(CRT)溶解再通作用效果,为抗阻运动辅助治疗CRT提供理论依据。方法:144只SD雄性大鼠随机分为CRT对照组、CRT+抗阻运动组、CRT+血红素氧合酶-1(HO-1)激动剂钴原卟啉(COPP)组、CRT+HO-1抑制剂锡原卟啉(SnPP)组,每组36只。构建大鼠CRT模型,置管10天后进行抗阻运动、腹腔注射COPP及SnPP处理,CRT对照组不做处理。干预后1、4、7、10、14、28天每组分别取6只大鼠,采用HE染色观察血栓溶解再通过程,免疫组化CD31染色检测新生血管,ELISA测定血清HO-1、白介素(IL)-6、IL-10含量,qPCR检测HO-1 mRNA相对表达量。结果:与CRT对照组相比,CRT+COPP组各时间点血栓溶解率升高、CRT+SnPP组降低,CRT+抗阻运动组第28天血栓溶解率高于CRT对照组,差异均有统计学意义(P<0.05,P<0.01)。ELISA结果显示,与CRT对照组相比,CRT+COPP组各时间点血清HO-1及IL-10含量上升,IL-6含量下降,差异均有统计学意义(P<0.05,P<0.01);而CRT+Snpp组趋势与其相反;CRT+抗阻运动组只在第28天血清HO-1及IL-10含量升高、IL-6含量下降,差异均有统计学意义(P<0.01)。而第28天,CRT+COPP组血清HO-1、IL-10及IL-6含量与CRT+抗阻运动组相比,差异无统计学意义(P>0.05)。q PCR结果表明,与CRT对照组相比,CRT+COPP组各时间点HO-1 mRNA表达升高,CRT+SnPP组表达降低,差异均有统计学意义(P<0.05,P<0.01);CRT+抗阻运动组第28天HO-1 mRNA升高至与CRT对照组差异有统计学意义(P<0.01),而CRT+COPP组与CRT+抗阻运动组相比差异无统计学意义(P>0.05)。免疫组化结果显示,与CRT对照组比较,CRT+COPP组中CD31表达增加,CRT+SnPP组表达降低,差异均有统计学意义(P<0.01)。CRT+抗阻运动组干预第28天CD31表达增加,与CRT对照组相比差异有统计学意义(P<0.01),而与CRT+COPP组相比差异无统计学意义(P>0.05)。结论:抗阻运动28天可上调HO-1表达,发挥抗炎、促血管新生作用,加速CRT溶解再通。Objective To explore the effect of resistance exercise accelerating dissolution and recana⁃lization of catheter-related thrombosis(CRT),so as to provide a theoretical basis for such a new treat⁃ment.Methods One hundred and forty-four male Sprague-Dawley rats were randomly divided into a CRT control group,a CRT+resistance exercise group,a CRT+serum heme oxygenase-1(HO-1)ago⁃nist cobalt protoporphyrin(COPP)group,and a CRT+HO-1 inhibitor tin protoporphyrin(SnPP)group,each of 36.Ten day after all rats were induced CRT,the CRT+resistance exercise group,COPP group and SnPP group were subjected to resistance exercise,intraperitoneal COPP and SnPP injection respectively.On the 1^(st),4^(th),7^(th),10^(th),14^(th) and 28^(th) day after the intervention,6 rats were killed and given hematoxylin eosin staining to observe the process of thrombus dissolution and recanalization.Meanwhile,immunohistochemical CD31 staining was conducted to detect neovascularization,and en⁃zyme-linked immune-sorbent assay was employed to detect the contents of HO-1,interleukin-6(IL-6)and interleukin-10(IL-10).Moreover,qPCR was used to detect the relative expression of HO-1 mRNA.Results Compared with the CRT control group,the average thrombus dissolution rate of the CRT+COPP group increased significantly,but that of the CRT+SnPP group decreased significantly at each time point.However,the average thrombus dissolution rate of the CRT+resistance exercise group was significantly higher than the CRT control group only on the 28^(th) day.According to the ELISA re⁃sults,compared with the CRT control group,the average serum HO-1 and IL-10 levels of the CRT+COPP group increased significantly,but the average IL-6 level decreased significantly at all time points(P<0.05).However,the average serum HO-1 and IL-10 levels of the CRT+SnPP group decreased sig⁃nificantly,but the average IL-6 levels increased significantly at all time points compared with the CRT control group.Meanwhile,the average serum HO-1 and IL-10 levels of the CRT+resistance exer⁃
