摘 要:目的 探讨人体尿液对羟基苯甲酸酯现状,分析其与肥胖、代谢综合征相关性。方法 采用便利抽样法,于2019年8月-2020年10月选取在南京市某健康管理中心进行健康体检的861例人员为研究对象。调查其一般资料、尿液(对羟基苯甲酸甲酯、对羟基苯甲酸乙酯、对羟基苯甲酸丙酯和对羟基苯甲酸丁酯浓度)、血液(高密度脂蛋白胆固醇、甘油三酯、空腹血糖值)。采用多变量线性回归模型分析经校正的对羟基苯甲酸酯浓度与肥胖、代谢综合征及其组成部分(腰围、高密度脂蛋白胆固醇、甘油三酯、空腹血糖和血压)之间的相关性,并按年龄、性别分层分析。结果 研究对象尿液样本对羟基苯甲酸甲酯、乙酯、丙酯和丁酯检测率分别为93%、43%、86%、17%;成人尿液对羟基苯甲酸酯几何平均值(GM:19.8μg/L)高于儿童(GM:7.2μg/L),成年女性(GM:29.8μg/L)高于男性(GM:9.5μg/L);在成年女性中,对羟基苯甲酸甲酯与肥胖、代谢综合征、腰围和高密度脂蛋白胆固醇间呈负相关(P<0.05),在成年男性中,对羟基苯甲酸丙酯浓度与代谢综合征患病率呈正相关(P<0.05)。结论 人体暴露于对羟基苯甲酸酯水平较高,与女性肥胖、代谢综合征、腰围和高密度脂蛋白胆固醇间呈负相关,与男性代谢综合征发生呈积极相关。Objective To investigate the current situation of p-hydroxybenzoate in human urine and analyze its correlation with obesity and metabolic syndrome.Methods By using the method of convenient sampling,861persons who underwent physical examination in a health management center in Nanjing from August 2019to October 2020were selected as subjects.The general data,urine(methyl p-hydroxybenzoate,ethyl p-hydroxybenzoate,propyl p-hydroxybenzoate and butyl p-hydroxybenzoate),blood(high density lipoprotein cholesterol,triglyceride,fasting blood glucose value)were investigated.Multivariate linear regression model was used to analyze the correlation between corrected phydroxybenzoate concentration and obesity,metabolic syndrome and its components(waist circumference,high density lipoprotein cholesterol,triglyceride,fasting blood glucose and blood pressure).And stratified analysis according to age and sex.Results The detection rates of methyl p-hydroxybenzoate,ethyl ester,propyl ester and butyl ester in urine samples were 93%,43%,86% and 17%,respectively.The geometric mean value(GM:19.8μg/L)of p-hydroxybenzoate in adult urine(GM:19.8μg/L)was higher than that in children(GM:7.2μg/L),and the detection rate of adult female(GM:29.8μg/L)was higher than that of male(GM:9.5μg/L).In adult women,methyl p-hydroxybenzoate was negatively correlated with obesity,metabolic syndrome,waist circumference and high density lipoprotein cholesterol(P<0.05).The concentration of propyl p-hydroxybenzoate was positively correlated with the prevalence of metabolic syndrome(P<0.05).Conclusion The level of human exposure to p-hydroxybenzoate is higher,which is negatively correlated with female obesity,metabolic syndrome,waist circumference and high density lipoprotein cholesterol,and positively correlated with male metabolic syndrome.
