“衬裙”Bentall在白塞病-主动脉瓣病变患者中的临床应用

摘　　要：目的探讨“衬裙”Bentall在白塞病-主动脉瓣病变患者中的临床应用及疗效。方法选取2016年09月至2019年12月武汉亚洲心脏病医院收治的白塞病-主动脉瓣病变患者8例,均行“衬裙”Bentall手术治疗,收集所有患者的围术期临床资料,并进行超声心动图随访。结果本组患者中4例患者为再次手术治疗,均为主动脉瓣置换术后,重度瓣周漏。所有患者均行“衬裙”Bentall手术治疗,手术成功,无围术期死亡。其中体外循环时间为(95.75±26.7)min,主动脉阻断时间为(69.37±17.23)min;1例患者术后引流多而行开胸探查;所有患者手术中均安置心脏表面临时起搏导线,1例患者术后早期出现Ⅲ度AVB,术后1周时恢复,随访过程中未再出现Ⅲ度AVB。所有患者均进行心脏彩超随访,随访期间瓣膜功能良好,无瓣周漏等并发症。结论“衬裙”Bentall治疗白塞病-主动脉瓣病变是安全有效的,可有效的预防瓣周漏的发生,近期随访临床疗效良好。Objective To explore the clinical results and application of“Chimney”Bentall procedure in patients with aortic incompetence caused by Behcet’s Disease.Methods From September 2016 to December 2019,8 patients underwent“Chimney”Bentall procedure in Wuhan Asia Heart Hosptial.The data were collected including the following-up data by echocardiogram and respectively analyzed to evaluate the clinical outcomes.Results Four patients underwent aortic valve replacement before and presented in our center for severe perivalvular leakage.All the patients underwent successful“Chimney”Bentall procedure without death in the hospital.The CPB and CCB time was(95.75±26.7)mins and(69.37±17.23)mins,respectively.One patient needed a reoperation for stopping bleeding.Temporary pacing lead was implanted during the procedure.Ⅲdegree AVB presented in one patient in early post-operation and recovered a week later.The heart rhythm was normal during the following-up.The function of aortic prosthesis was normal without perivalvular leakage during the echocardiogram following-up.Conclusions With a good recent following-up,“Chimney”Bentall procedure could be used in the patients with with aortic incompetence caused by Behcet’s Disease safely and effectively,which could reduce the perivalvular leakage with excellent outcomes.
