论“戾嗜”在疫病辨治中的价值

摘　　要：疫病不同于时病,时疫也不能涵盖所有的疫病。新型冠状病毒肺炎(简称“新冠肺炎”)疫情前所未有的时空跨度,给传统中医疫病理论带来了诸多挑战,如非时疫类疫病的中医属性如何判定、无症状感染者的防治方案如何制定等。针对这些问题,结合中医学对新冠肺炎的临床实践和对既往疫病“因机证治”的综合分析,发现“戾嗜”(戾气对理化环境的亲嗜性)是解决这些问题的重要突破口。通过对相关病原体“戾嗜”的判定,可以为“非风、非寒、非暑、非湿”的戾气赋予中医学属性,进而为疫病病性的判定、病机主线的判定以及无症状感染者防治方案的制定等提供切入点。Epidemic diseases are different from seasonal diseases,and seasonal epidemic cannot cover all epidemic diseases.The unprecedented temporal and spatial variation of the coronavirus disease 2019(COVID-19) has brought many challenges to the traditional theories of epidemic diseases in traditional Chinese medicine(TCM),such as how to determine the TCM attributes of non-seasonal epidemic diseases,and how to formulate prevention and treatment protocols for asymptomatic infections. "Epidemic pathogen tropism"(the tropism of epidemic pathogen to the physical and chemical environment) has been found to be the breakthrough,by referring to the clinical experience of TCM in the management of COVID-19 cases and analyzing the "mechanism,syndrome and treatment" of the previous epidemic diseases.By judging the "epidemic pathogen tropism" of the relevant pathogens,the epidemic qi that is "non-wind,non-cold,non-summerheat,and non-damp" can be given TCM attributes,thereby providing the entry point for the determination of the nature of epidemic diseases and the main disease mechanism,and the development of prevention and treatment protocols for asymptomatic infections.
