摘  要:目的探究细胞学快速现场评价(C-ROSE)技术联合支气管镜检查及经皮肺穿刺活检在肺占位性疾病诊断中的价值。方法本研究为病例对照研究。采用非随机抽样的方法收集喀什地区第一人民医院2019年6月至2021年6月收治的100例周围性肺占位病变患者的临床资料。依据其是否采用C-ROSE技术分为A、B组,A组38例患者采用支气管镜检查及经皮肺穿刺活检,B组62例患者在A组检查基础上加用C-ROSE技术。比较2组患者对肺占位性疾病的诊断结果,比较2组患者穿刺次数、穿刺时间、住院时间、住院费用、并发症发生情况等。结果B组患者诊断符合率为91.94%(57/62)高于A组76.32%(29/38)(χ^(2)=4.77,P=0.029)。B组患者穿刺时间、穿刺次数、住院时间、住院费用均低于A组(P值均<0.05)。B组并发症总发生率(4.84%,3/62)较A组(13.16%,5/38)低,差异无统计学意义(χ^(2)=2.21,P=0.137)。结论C-ROSE技术联合联合支气管镜检查及经皮肺穿刺活检能够有效提高周围性肺占位病变的诊断符合率,且减少穿刺时间及次数、住院时间与住院费用,降低并发症的发生率,值得临床参考借鉴。Objective To explore the value of rapid on-site evaluation of cytology(C-ROSE)technique combined with bronchoscopy and percutaneous lung biopsy in the diagnosis of lung space-occupying diseases.Methods The present study is a case-control study.The non-random sampling method was used to collect clinical data of 100 patients with peripheral lung space-occupying lesions admitted to The First People′s Hospital of Kashgar from June 2019 to June 2021,and the patients were divided into Group A and Group B based on the availability of C-ROSE technology.The Group B(n=62)received bronchoscopy+percutaneous lung biopsy+C-ROSE technology,and the Group A(n=38)only received the first two.The diagnosis results of lung space-occupying diseases of patients in two groups were compared,and the puncture frequency,puncture duration,length of stay,hospitalization cost and complications of the two groups were compared.Results The diagnostic coincidence rate in the Group B was higher than that in the group A(91.94%[57/62]vs 76.32%[29/38],χ^(2)=4.77,P=0.029,respectively).The puncture duration,puncture frequency,the length of stay and hospitalization expenses in the Group B were significantly lower than those in the Group A(P<0.05 at all points).The total incidence of complications in the Group B was lower than that in the Group A(4.84%[3/62]vs 13.16%[5/38]),and the difference was not statistically significant(χ^(2)=2.21,P=0.137).Conclusions C-ROSE technology combined with bronchoscopy and percutaneous lung biopsy is capable of effectively improving the diagnostic coincidence rate of peripheral lung occupying lesions,significantly reducing the puncture duration and frequency,the length of stay and hospitalization expenses,and decreasing the incidence of complications,being worthy of clinical reference.
