摘  要:目的观察循经通络推拿手法结合药物治疗与单纯手法、单纯药物治疗对乳腺增生(MGH)大鼠的疗效差异及对下丘脑-垂体-卵巢(HPO)轴的影响。方法SD大鼠随机分为对照组、模型组、手法组、药物组、药物加手法组,每组10只。采用孕雌激素联合造模法制备乳腺增生症模型。手法组予以循经通络推拿手法治疗,药物组灌胃给予三苯氧胺,药物加手法组同时给予循经通络推拿和药物灌胃。治疗结束后测量大鼠乳头直径和乳头高度,HE染色观察乳腺组织病理学变化,ELISA法检测血清中卵泡刺激素(FSH)和黄体生成素(LH)水平,Western-blot及RT-RCR检测下丘脑促性腺激素释放激素(GnRH)及垂体促性腺激素释放激素受体(GnRH-R)的蛋白和mRNA表达。结果与模型组比较,各治疗组大鼠乳头直径、乳头高度显著减小(P<0.05);药物加手法组大鼠乳腺组织病理形态明显改善,手法组和药物组大鼠乳腺组织形态改善不明显;药物组、药物加手法组血清FSH水平明显降低,LH水平显著升高(P<0.05),手法组大鼠血清FSH、LH水平变化不明显(P>0.05);药物加手法组大鼠GnRH、GnRH-R的蛋白和mRNA表达均明显减少(P<0.05),药物组明显下调了GnRH和GnRH-R的mRNA表达,而对其蛋白水平未见明显调控作用(P>0.05),手法组GnRH、GnRH-R的蛋白表达和mRNA表达均无明显变化(P>0.05)。结论循经通络推拿手法结合三苯氧胺对乳腺增生症大鼠的疗效优于单纯手法、单纯药物治疗,其作用机制可能与降低下丘脑GnRH和垂体GnRH-R的表达,影响下丘脑-垂体-卵巢轴功能相关。Objective To observe the difference in curative effects between a massage manipulation along meridians combined with a drug therapy,a massage manipulation and a drug therapy on rats with mammary gland hyperplasia(MGH)and its effect on the hypothalamus-pituitary-ovary(HPO)axis.Methods SD rats were randomly divided into a control group,a model group,a manipulation group,a drug group,and a drug plus manipulation group,with 10 rats in each group.The MGH rat model was established with progesterones.The manipulation group was treated with massage manipulations along meridians,while the drug group was treated with tamoxifen by gavage,and the drug plus manipulation group was given both,all for 30 days.After treatment,the nipple diameters and nipple heights of the rats were measured,the histopathological changes of mammary glands were observed by HE staining,the levels of follicle-stimulating hormone(FSH)and luteinizing hormone(LH)in serum were detected by ELISA,and the protein and mRNA expressions of the hypothalamic gonadotropin releasing hormone(GnRH)and pituitary gonadotropin releasing hormone receptor(GnRH-R)were detected by Western blot and RT-RCR.Results Compared with the model group,the nipple diameters and nipple heights of the rats in each treatment group were significantly reduced(P<0.05),the pathological morphology of the mammary gland tissues in the drug plus manipulation group was significantly improved,while it was not significantly improved in the manipulation group and the drug group.The serum FSH levels in the drug group and the drug plus manipulation group were significantly decreased,and the LH levels were significantly increased(P<0.05),while the serum FSH and LH levels of the rats in the manipulation group did not change significantly(P>0.05).The protein and mRNA expressions of GnRH and GnRH-R were significantly decreased in the drug plus manipulation group(P<0.05),and the drug group significantly down-regulated the mRNA expressions of GnRH and GnRH-R,but had no significant regulatory effect on their
