摘 要:目的探讨胆固醇水平与2型糖尿病中青年男性骨代谢的相关性。方法选择79例2型糖尿病中青年男性,根据总胆固醇(total cholesterol,TC)水平分为正常TC组(TC<4.5 mmol/L)及高TC组(TC≥4.5 mmol/l/L),检测所有患者的血脂[TC、甘油三脂(triglyceride,TG)、高密度脂蛋白胆固醇(high density lipoprotein-cholesterol,HDL-C)、低密度脂蛋白胆固醇(low density lipoprotein-cholesterol,LDL-C)]、糖化血红蛋白(HbA1c)及骨代谢标志物[甲状旁腺素(parathyroid hormone,PTH)、25羟维生素D(25-OH-D)、骨钙素(osteocalcin,OC)、血清Ⅰ型前胶原N-末端前肽(P1NP)、Ⅰ型胶原交联C-末端肽(β-CTX)]和骨密度(bone mineral density,BMD),分析血脂与骨密度及骨代谢标志物间的相关性。结果校正年龄、糖化血红蛋白、BMI的影响后,2型糖尿病中青年男性患者胆固醇水平与腰椎(L_(1~4))BMD呈负相关(r=−0.266,P=0.034),LDL-C水平与PTH呈正相关(r=0.244,P=0.042)。结论高胆固醇血症与2型糖尿病中青年男性腰椎骨密度降低有关且独立于年龄、BMI及血糖控制情况。Objective:To investigate the correlation between total cholesterol level and bone metabolism in young and middle-aged patients with type 2 diabetes.Methods:Seventy-nine young and middle-aged patients with type 2 diabetes were divided into two groups,according to TC level:high cholesterol group(TC≥4.5 mmol/l/L)and normal cholesterol level group(TC<4.5 mmol/L)The serum lipids[total cholesterol(TC),triglyceride(TG),high density lipoprotein cholesterol(HDL-C),low density lipoprotein cholesterol(LDL-C)],glycated hemoglobin and bone mineral density(BMD),and bone metabolism markers were detected and correlation between blood lipid,bone mineral density and bone metabolic markers was analyzed.Results:After the adjusting of age,HbA1c and BMI,cholesterol level was negatively correlated with lumbar bone mineral density in young and middle-aged men with Type 2 diabetes(r=−0.266,P=0.034)and LDL-C level is positively correlated with PTH(r=0.244,P=0.042).Conclusion:The high cholesterol level is associated with decreased lumbar bone density in young and middle-aged patients with Type 2 diabetes and is independent of age,BMI,and glycemic control.
