摘　　要：目的观察术中分别输注琥珀酰明胶和羟乙基淀粉对行妇科腹部大手术患者术后恶心呕吐情况的不同影响。方法纳入120例行妇科腹部大手术的患者,随机分为3组,分别是勃脉力(复方电解质注射液)组40例、佳乐施(琥珀酰明胶注射液)组40例和万汶(羟乙基淀粉130/0.4氯化钠注射液)组40例,各组患者在手术开始后的1小时内输注15 ml/kg相应的液体,随后再根据心率和血压的需要输注勃脉力溶液维持。结果与勃脉力组相比,佳乐施组和万汶组术后的恶心VAS评分均显著下降(P<0.05),但术后24 h时佳乐施组的评分略高于万汶组(P<0.05);三组的术后呕吐发生率没有显著差异(P>0.05)。结论术中输注琥珀酰明胶和羟乙基淀粉的输液策略都能有效改善患者术后恶心呕吐的情况,其中使用后者的改善效果更加持久。Objective To observe the different effects of intraoperative infusion of succinyl gelatin and hydroxyethyl starch on postoperative nausea and vomiting in patients undergoing gynecological major abdominal surgery.Methods A total of 120 patients who underwent gynecological major abdominal surgery were randomly divided into 3 groups,namely,multiple electrolytes injection group(40 cases),succinyl gelatin group(40 cases)and hydroxyethyl starch group(40 cases);patients in each group were infused with 15 ml/kg of the fluid mentioned above within 1 hour after the operation started,and then infused with multiple electrolytes injection according to hemodynamics.Results Compared with the multiple electrolytes injection group,the VAS scores of nausea in the succinyl gelatin group and hydroxyethyl starch group decreased significantly(P<0.05),but the score of the succinyl gelatin group was slightly higher than that of the hydroxyethyl starch group at 24 hours after surgery(P<0.05).There was no significant difference in the incidence of postoperative vomiting among the three groups(P>0.05).Conclusion The intraoperative infusion strategy of succinyl gelatin and hydroxyethyl starch can effectively improve the postoperative nausea and vomiting of patients,and the improvement effect of the latter is more lasting.
