摘  要:目的:探究并分析胎盘生长因子(PLGF)修复大鼠颅骨缺损的效果及对新骨形成的影响。方法:将15只SD大鼠随机分为A组(空白对照组)、B组(胶原/羟基磷灰石组)、C组(含PLGF的胶原/羟基磷灰石组),每组5只。构建颅骨缺损大鼠模型,造模成功后A组大鼠颅骨缺损处不做任何处理,B组大鼠颅骨缺损区植入8 mm×1.5 mm的胶原/羟基磷灰石支架,C组大鼠颅骨缺损处植入8 mm×1.5 mm含有PLGF的胶原/羟基磷灰石支架。造模后8周处死各组大鼠,取颅骨损伤部位行Micro CT扫描、生物力学测试和病理组织学染色,对比各组大鼠骨体积(BV)、骨矿物质密度(BMD)、骨体积/组织体积(BV/TV)等骨组织形态计量指标差异,采用实时荧光定量聚合酶链式反应法检测各组大鼠颅骨损伤标本中成骨相关基因Ⅰ型胶原(COLⅠ)、骨桥蛋白(OP)、骨钙素(OC)、Runt相关转录因子2(Runx2)的mRNA相对表达水平。结果:Micro CT扫描结果显示,B组大鼠颅骨缺损程度优于A组,C组大鼠颅骨缺损程度明显小于A、B组,缺损基本愈合,可观察到大量新骨形成。病理组织学染色结果发现,B组病理损伤程度轻于A组,C组大鼠组织病理损伤程度显著轻于A、B两组。B组BV、BMD、BV/TV等骨组织形态计量指标均优于A组(P<0.05),C组骨组织形态计量指标均优于A、B两组(P<0.05)。B组新生骨组织最大负载力值高于A组(P<0.05),C组新生骨组织最大负载力值较A、B两组明显偏高(P<0.05)。B组COLI、OP、OC、Runx2等成骨相关基因的mRNA相对表达水平均优于A组(P<0.05),C组大鼠成骨相关基因的mRNA表达水平均显著高于A、B两组(P<0.05)。结论:PLGF可有效促进颅骨缺损大鼠骨修复愈合过程,诱导新骨形成,有望成为极具潜力的临床新型颅骨缺损修复材料。Objective:To explore and analyze the effects of placental growth factor(PLGF)on repairing the skull defects in rats and its influence on new bone formation.Method:A total of 15 SD rats were randomly divided into group A(blank control group),group B(collagen/hydroxyapatite group)and group C(collagen/hydroxyapatite group with PLGF),with 5 rats in each group.The rat models of skull defects were constructed.After successful modeling,no treatment was performed on skull defect area of rats in group A,8 mm×1.5 mm collagen/hydroxyapatite scaffold was implanted in skull defect area of rats in group B,8 mm×1.5 mm collagen/hydroxyapatite scaffold containing PLGF was implanted in skull defect area in group C.Rats in each group were sacrificed at 8 weeks after modeling,the damaged parts of skull were taken for Micro CT scan,biomechanical testing and histopathological staining.Bone histomorphometric indicators such as bone volume(BV),bone mineral density(BMD)and bone volume/tissue volume(BV/TV)were compared.Quantitative real-time polymerase chain reaction was used to detect the mRNA relative expression levels of osteogenesis-related genes of typeⅠcollagen(COLⅠ),osteopontin(OP),osteocalcin(OC)and Runt related transcription factor 2(Runx2).Result:Micro CT scan showed that the degree of skull defects in group B was better than that in group A,and the degree of skull defects in group C was significantly smaller than that in group A and group B.The defects were basically healed and a large number of new bone formation could be observed.Histopathological staining showed that the degree of pathological damage in group B was milder than that in group A,and the degree in group C was significantly milder than that in group A and group B(P<0.05).The bone histomorphometric indicators(BV,BMD and BV/TV)in group B were better than those in group A(P<0.05),and those in group C were better than those in group A and group B(P<0.05).The maximum load of new bone tissue in group B was higher than that in group A(P<0.05),and the maximum load
