摘  要:目的:探究食管癌患者进行根治术治疗后早期的营养护理方法,分析早期营养护理对患者胃肠功能、营养状况的改善效果。方法:选取南通市肿瘤医院肿瘤外科2020年6月-2021年5月收治的120例食管癌根治术患者,采用随机数字表法将患者分为观察组与对照组。两组患者均接受常规护理,观察组在此基础上行早期营养护理。对两组患者的胃肠功能、营养状况进行对比。结果:护理前两组患者的血清白蛋白(Alb)、前白蛋白(PAB)比较,差异均无统计学意义(P>0.05);护理7 d后,两组患者的以上指标均有明显改善,观察组的改善效果更加显著(P<0.05)。观察组的首次排气时间、首次肠鸣音时间、首次排便时间均早于对照组(P<0.05)。观察组的营养不耐受发生率为21.67%,低于对照组的46.67%(P<0.05)。结论:在食管癌根治术后,根据患者个体的营养状况,参考营养不良的五阶梯原则,规范制订早期营养护理方案,并于患者体征基本稳定后尽早实施营养支持治疗,能够有效改善患者的营养状况,促进其胃肠功能的恢复,且有降低患者营养不耐受风险等积极作用。Objective:To explore the early nutritional nursing methods for patients with esophageal cancer after radical surgery,and to analyze the effect of early nutritional nursing on the improvement of gastrointestinal function and nutritional status.Method:A total of 120 patients who underwent radical esophagectomy in the Department of Oncology of Nantong Cancer Hospital from June 2020 to May 2021 were randomly divided into observation group and control group.Both groups received routine nursing,the observation group received early nutritional nursing on this basis.The gastrointestinal function and nutritional status of the two groups were compared.Result:There were no significant differences in serum albumin(Alb)and prealbumin(PAB)between the two groups before nursing(P>0.05);after 7 days of nursing,the above indexes of the two groups were significantly improved,and the improvement effect of the observation group was more significant(P<0.05).The first exhaust time,the first bowel sound time and the first defecation time of the observation group were earlier than those of the control group(P<0.05).The incidence of nutritional intolerance in the observation group was 21.67%,lower than 46.67%in the control group(P<0.05).Conclusion:After radical resection of esophageal cancer,according to the individual nutritional status of patients,referring to the five ladder principle of malnutrition,standardizing the formulation of early nutritional nursing plan and implementing nutritional support treatment as soon as possible after the patients’physical signs are basically stable can effectively improve the nutritional status of patients and promote the recovery of gastrointestinal function,with a positive effect on reducing the risk of nutritional intolerance.
