磁共振成像联合视诱发电位和视野检查对球后视神经炎的诊断价值

摘　　要：目的:探讨磁共振成像(magnetic resonance imaging,MRI)、视觉诱发电位(visual evoked potential,VEP)和视野检查对球后视神经炎(retrobulbar optic neuritis,RN)的诊断价值。方法:回顾性分析2016年5月~2022年1月本院收治的38例RN患者,分别行MRI、VEP及视野检查,对比三种检查方法对RN的检出阳性率,分析P100波、P2波振幅和峰时及视野的平均缺损等指标对RN的诊断作用。结果:三种方法对RN的检出阳性率从高到低依次为视野、VEP、MRI。RN组的P100波、P2波的峰时相对正常组均延迟(P<0.05),P100波、P2波振幅相对正常组均下降(P<0.05),RN组视野的MD低于正常组(P<0.05),差异均有统计学意义。结论:三种检查各有自身的优势与不足,联合三者的检查可以多维度的对RN进行全面评估,对于RN的诊断有重要意义。Objective:To explore the diagnostic value of magnetic resonance imaging,visual evoked potential and visual field examination in retrobulbar optic neuritis.Methods:A retrospective analysis of 38 patients with RN who were admitted to our hospital from May 2016 to January 2022,underwent MRI,VEP and visual field examinations,respectively,compared the positive rates of RN detection by the three examination methods,and analyzed the amplitude of P100 wave and P2 wave.The diagnostic effect of indicators such as peak time and average defect of visual field on RN.Results:The positive rates of the three methods for RN detection were visual field,VEP,and MRI in descending order.The peak time of P100 wave and P2 wave in RN group were delayed compared with the normal group(P<0.05),and the amplitudes of P100 wave and P2 wave were decreased compared with the normal group(P<0.05).The MD of the visual field in the RN group was lower than that in the normal group(P<0.05),the difference was statistically significant.Conclusion:Each of the three inspections has its own advantages and disadvantages,and the combined inspection of the three can comprehensively evaluate RN in multiple dimensions,which is of great significance for the diagnosis of RN.
