摘 要:目的:探讨胎盘三维能量多普勒指数(three-dimensional power Doppler index,3D-PDI)在子痫前期(preeclampsia,PE)孕妇中的应用价值。方法:选择2018年5月~2021年8月就诊于本院晚孕期孕妇的资料,122例孕妇分为子痫前期组(观察组,60例)和正常妊娠组(对照组,62例)。用三维能量多普勒超声(three-dimensional power Doppler ultrasound,3D-PDU)并通过超声活检法获得的胎盘3D-PDI—血管化指数(vascularization index,VI)、血流指数(flow index,FI)和血管化血流指数(vascularization flow index,VFI)来定量监测胎盘血流灌注情况;CD34单克隆抗体染色法计数胎盘微血管密度(microvessel density,MVD);比较两组间的各项差异,并对3D-PDI(VI、FI、VFI)与MVD之间的关系进行多因素线性回归分析。结果:观察组孕妇体质量指数(body mass index,BMI)明显高于对照组(P<0.05);观察组孕妇分娩孕周、VI、FI、VFI及MVD均明显低于对照组(P<0.05);胎盘3D-PDI(VI、FI、VFI)与MVD均成正相关,且以VFI相关性最佳,三者B值分别为7.758、1.276、15.903。结论:超声活检法的应用扩大了3D-PDU的适用范围,使得晚孕期的胎盘3D-PDI(VI、FI、VFI)更加准确方便快捷监测子痫前期孕妇胎盘的血流情况,为临床诊疗提供重要参考价值。Objective:To investigate the application value of placental three-dimensional power Doppler index(3D-PDI)in pregnant patients with preeclampsia(PE).Methods:According to the data of pregnant women in the third trimester who were treated in our hospital from May 2018 to August 2021,122 pregnant women were divided into a preeclampsia group(observation group,n=60)and a normal pregnancy group(control group,n=62).Placenta 3D-PDI—vascularization index(VI),flow index,(FI)and vascularization flow index(VFI)were obtained by three-dimensional power Doppler ultrasound(3D-PDU)through ultrasound biopsy method to quantitatively monitor placental blood perfusion;Placental microvessel density(MVD)was counted by CD34 monoclonal antibody staining;The differences between the two groups were compared,and the multivariate linear regression analysis was performed on the relationship between 3D-PDI(VI,FI,VFI)and MVD.Results:The body mass index(BMI)of pregnant women in the observation group was significantly higher than that in the control group(P<0.05);The delivery gestational week,placental 3D-PDI(VI,FI,VFI)and MVD of pregnant women in the observation group were significantly lower than those in the control group(P<0.05);The placental 3D-PDI(VI,FI,VFI)were all positively correlated with microvessel density,and VFI had the best correlation,with the B values of 7.758,1.276,15.903,respectively.Conclusion:The application of ultrasound biopsy method has expanded the scope of application of 3D-PDU,making placental 3D-PDI(VI,FI,VFI)in the third trimester more accurate,convenient and quick to monitor the blood flow of the placenta in pregnant women with preeclampsia,providing an important reference value for clinical diagnosis and treatment.
