覆膜支架治疗肾下型腹主动脉瘤的效果观察

摘　　要：目的:分析肾下型腹主动脉瘤疾病特点,评价覆膜支架治疗取得的效果,为血管外科手术治疗工作提供参考。方法:选择本院血管外科2018年12月~2019年8月的80例肾下型腹主动脉瘤疾病患者,进行分组手术治疗。观察组40例患者采取覆膜支架腔内修复术治疗,对照组40例患者采取传统开腹手术治疗,医院伦理委员会审核批准。比较2组受试肾下型腹主动脉瘤患者的治疗情况,包括围术期指标与相关并发症发生情况。结果:组间围术期指标比较,观察组患者的手术时间(214.50±20.50)min、术中出血量(121.50±15.50)mL、术中输血量(35.50±5.50)mL、ICU观察时间(1.50±0.20)d、首次下床活动(6.20±1.20)d、住院时间(9.50±1.50)d均明显少于对照组,P<0.05;组间术后切口感染等术后并发症发生情况比较,观察组与对照组并发症总发生率差异显著,P<0.05。结论:相较于开腹手术治疗,覆膜支架治疗肾下型腹主动脉瘤疾病具有明显的优势,手术损伤小、术后恢复快、并发症少。Objective:To analyze the characteristics of subrenal abdominal aortic aneurysm disease,evaluate the effect of stent graft treatment,and provide a reference for vascular surgery.Methods:Select 80 patients with subrenal abdominal aortic aneurysm disease from December 2018 to August 2019 in the Department of Vascular Surgery in our hospital for group surgery.40 patients in the observation group were treated with endovascular repair with covered stents,and 40 patients in the control group were treated with traditional open surgery,which was reviewed and approved by the hospital ethics committee.The treatment of the two groups of patients with subrenal abdominal aortic aneurysm were compared,including perioperative indicators and related complications.Results:Comparison of perioperative indicators between groups,observation group patients’operation time(214.50±20.50)min,intraoperative blood loss(121.50±15.50)mL,intraoperative blood transfusion(35.50±5.50)mL,ICU observation time(1.50±0.20)d,first getting out of bed(6.20±1.20)d,and hospitalization time(9.50±1.50)d were significantly less than those of the control group,P<0.05;Comparison of postoperative complications such as infection of incisions between groups,There was a significant difference in the total incidence of complications between the observation group and the control group,P<0.05.Conclusion:Compared with open surgery,the covered stent has obvious advantages in the treatment of subrenal abdominal aortic aneurysm disease.It has less surgical damage,faster postoperative recovery,and fewer complications.
